冠心病病人治疗前后血浆溶血磷脂酸水平变化的研究

摘　　要：目的探讨冠心病病人血浆溶血磷脂酸(LPA)水平的临床意义。方法选择隐匿型冠心病组(I组)40例;心绞痛型冠心病组(Ⅱ组)30例;心肌梗死型冠心病(AMI)组(Ⅲ组)45例;缺血性心肌病组(IV组)36例;猝死型冠心病组(V组)31例,并选择60例健康者作为正常对照组,分别测定LPA。各组受检者2周后复查LPA。结果冠心病各患者组(I-V组)及对照组初查时血浆LPA浓度分别为2.96±l.44mol/L、3.02±0.76μmol/L、3.80±0.63μmol/L、2.71±0.84μmol/L、3.63±l.51μmol/L、1.84±0.67μmol/L,各组间LPA结果差异显著(P<0.01),且均高于对照组(P<0.01)。治疗2周后各组LPA较治疗前下降显著(P<0.01),治疗后除I-IV;Ⅱ-V组之间LPA浓度差异无显著性(P>0.05)外,其余各组间LPA浓度差异显著。且各组治疗后与对照组比较LPA浓度差异显著(P<0.001)。结论提示血浆LPA作为体内凝血和血栓形成过程早期释放的分子标记物,在预测冠心病病人的发生、转归及指导合理应用抗血小板及抗凝药物方面有重要应用价值。Objective To investigate clinical significance of the changes of plasma lysophosphatidic acidLPA) levels in patients with Coronary Heart Disease.Methods 182 patients with Coronary Heart Disease were divided into 5 groups:Group of Latent Coronary Heart DiseaseGroup I),40 Group of Angina Coronary Heart DiseaseGroup Ⅱ)30,Group of Myocardial Infarction Coronary Heart DiseaseGroup Ⅲ)45,Group of is chemic CardiomyopathyGroup IV)36,Group of Sudden Death Coronary Heart DiseaseGroup V)31,and compared with matched healthy Cointrol group 60,the plasma LPA levels of them were respectively determined two times.Result The plasma LPA levels before therapyGroup ⅠⅡ Ⅲ IV V and control group)were:2.96±l.441μmol/L,3.02±0.76μmol/L,3.80±0.63μmol/L,2.71±0.84μmol/L,3.63±1.51μmol/L,1.84±0.67μmol/L.The LPA Levels of all the 5 groups have significant changes to each otherp<0.01),and they are all higher than that of control group.After two weeks therapy,the LPA levels of the 5 groups decreased greatlyp<0.01);all of them have signiflcant changes to each other except Group I to Group IV and Group Ⅱ to Group V.And after therapy,all of the 5 groups have changes on LPA levels to the control groupp<0.001).Conclusion As a molecular marker released by activated platelet in vivo,plasma LPA may be Valuable for predicting occurrence and development of Coronary Heart Disease and guiding anticoagulant treatment.
