摘 要:目的检测肾透析病人外周血单个核细胞(PBMC)和血浆标本中HHV-7DNA,探讨肾透析人群中HHV-7的感染及致病作用。方法采用巢式聚合酶链反应(Nested-PCR)对50例肾透析病人和54例正常健康人PBMC和血浆标本中HHV-7DNA进行定性检测;采用限制性核酸内切酶EcoR I对PCR产物进行酶切图谱分析以确定扩增产物的特异性。结果①50例肾透析病人PBMC中HHV-7DNA阳性率为82.0%(41/50),血浆中HHV-7DNA阳性率为16(8/50)。②54例健康献血员PBMC中HHV-7DNA阳性率为51.8%(28/54),血浆中HHv-7DNA阳性率为0%(0/54)。③统计学分析表明肾透析病人和健康人献血员PBMC标本中HHV-7阳性检出率有显著性差异X2=10.57,P<0.05;肾透析病人血浆可检测到HHV-7DNA,而健康献血员血浆标本中HHV-7DNA阴性,二者之间具有显著性差异(X2=9.36,P<0.05)。结论①肾透析病人外周血单个核细胞和血浆中HHV-7的感染率较高,提示肾透析过程中可能存在该病毒的交叉感杂或潜伏HHV-7的激活。②巢式PCR结合限制性酶切分析是检测HHV-7感染敏感、特异和有效的方法。③部分肾透析病人血浆中可检测到HHV-7DNA,提示这些病人体内存在HHV-7的活动性感染。Objective To investigate the prevalence of human herpesvirus 7(HHV-7);in peripheralblood of patients after treatment of artificial kidney,and todefine the infecti on and pathogencity of HHV-7in artificial Kidney patients.Methods In 50 cases of patients after treatment of artificialKidney and 54 healthy volunteers,the prevalence of HHV-7 in peripheral blood mononuclear cell and cell-free viral DNA in plasma was tested by nested polymerase chain reaction(PCR)assay;the product of PCR was digested by restricted enzyme EcoR I to identify its specificity.Results ①of the 50 patients,the prevalence of HHV-7 in peripheral blood mononuclear cells was 82%(41/50),theprevalence of HHV-7 in plasma was 16%(8/50).② of the 54 healthy individuals,the prevalence of HHV-7 in peripheral blood mononuclear cells was 51.8%(28/54),the prevalence of HHV-7 inplasma was 0.0%(0/54).③ There is a significant diffrence on the positive rate of HHV-7 in peripheral blood mononuclear cells between the patients and the healthy individuals(X^2=10.57,P<0.05).The diffrence of positive rate of HHV-7 in plasma was also significant between the patientsand the healthy individuals X^2=9.36,P<0.05.Conclusion ① These results indicated that there Was higher HHV-7DNA positive rate in the peripheral blood mononuclear cells of artificial kidneypatients,which indicates that cross infection might exist during the process of artificial kidney,or the poential HHV-7 was reactivated easily.② Nested-PCR combined with restriction enzyme technique is a sensitive,specific and effective method in detecting HHV-7.③ Some of artificial Kidneypatients were HHV-7DNA positive in plasma,while none of the controls was HHV-7 DNA positive,It suggested that active infection of HHV-7 might be related to the artificial kidney.
