手部皮肤撕脱伤皮瓣修复术后臃肿畸形的整形治疗

摘　　要：目的探讨手部皮肤撕脱伤皮瓣修复术后臃肿的一次整形手术治疗方法。方法本组15例手部皮肤撕脱伤皮瓣修复术后患者,术中在受区基底保留1mm～2mm厚的薄层筋膜脂肪层,将皮瓣全部修薄成全厚皮或者中厚皮重新游离植皮。结果本组15例患者手部皮瓣一次修薄整形术后回植皮全部成活,手部外形显著改善,手指接近正常形态。结论本方法适用于手部大面积皮肤撕脱伤和多指外伤皮瓣修复术后皮瓣臃肿的整形,方法简便,可通过一次手术达到手部外形最大限度的恢复。Objective To explore the plastic surgery treatment method about the bulky deformity after the flap repair of the hand avulsion injury. Methods 15 patients had been performed to preserve a lmm^2mm thickness of fascia-fatty tissue on the recipient site, remove all the subcutaneous tissue of the covered flap and replace the flap skin as a graft. Results The whole graft takes in 15 patients occurred uneventfully after operation, with the flap bulky appearance significantly reduced and the digital appearance reached ro nearly normal. Conclusion The procedure is indicated in the bulky deformity after the flap repair of the hand avulsion and multi-digits injury. It is easy to perform with a good appearance of the hand achieved.
