摘　　要：目的探讨双锁定钢板固定治疗股骨干粉碎骨折的疗效。方法采用双锁定钢板前、外侧固定股骨粉碎骨折15例,其中3例术前出现下肢深静脉血栓者安装静脉滤器后手术。结果手术时间为75～152(98±7.7)min。15例均获随访,时间8～30(10.6±2.2)个月。骨折愈合时间12～18(13.9±3.2)周。根据HSS膝关节临床功能评分标准:优10例,良3例,可2例。结论双锁定钢板对粉碎骨块起到良好的整复与固定,可早期进行邻近关节功能锻炼;既避免骨折局部的应力集中又提供适宜的应力环境,提供骨折愈合有利条件,治疗股骨干粉碎骨折疗效满意。Objective To assess the efficacy of the dual-locking plates for multifragmentary femoral shaft fracture.Methods 15 cases with comminuted fracture of the femoral shaft were treated by dual-locking plates fixation anteriorly and laterally,and 3 cases received inferior vena cava filter because of deep vein thrombosis before the operation.Results Operative timing was from 75 to 152 minutes(average 98±7.7minutes).All cases were followed up for 8 to 30 months(average 10.6±2.2months).The healing time ranged from 12 to 18 weeks(average 13.9±3.2 weeks).Their functional recovery was evaluated according to criteria by HSS,10 cases were rated as excellent,3 good and 2 fair.Conclusions Dual-locking plates can reduce and stabilize the comminuted fragments firmly,which allowed the knee joint to actively move earlier.It has better biomechanical properties and prevents stress concentration,provides optimal stress environment and conditions for fracture healing,and satisfactory results can be expected to treat comminuted femoral shaft fractures.
