摘  要:目的探讨兔髓核内注射转化生长因子-β1(TGF-β1)对椎间失稳后软骨终板组织形态学的影响。方法选6月龄日本大耳白兔36只,体重(2.5±0.2)kg,雌雄不限。随机分为预防组(18只)、对照组(18只)。全部实验动物均通过分离骶棘肌、切断L5~6、L6~7棘上、棘间韧带、关节囊及两侧关节突关节后外1/2,造成L5~6、L6~7椎间失稳模型。预防组动物在完成椎间失稳手术后立即经侧后方入路显露L5~6、L6~7椎间盘,髓核内注射规格为0.01 mg/ml TGF-β1 25~30μl。于术后第3、6个月分别对两组白兔各取8只采集其软骨终板组织,进行HE染色及Mankin评分,并用SPSS 15.0软件进行统计学分析。结果术后第3、6个月,预防组较对照组软骨终板软骨细胞分布均匀,潮线清晰;Mankin评分降低(P<0.05)。结论兔髓核内注射TGF-β1对椎间失稳后软骨终板退变具有明显的预防作用。Objective To investigate the effect of transforming growth factor-β1 on obviation of degeneration of the cartilage endplate due to the intercertebral destabilization by intra-nucleus gelatinosus injection.Methods 36 white macrotia rabbits of Japan with 6 months old were recruited,which weighed within the range of(2.5±0.2) kg,female and male was unrestricted.These rabbits were divided into 2 groups randomly,with 18 rabbits in the contrast group,with 18 rabbits in the preventive group.All of the experimental rabbits were destabilized by operation on L5-6 and L6-7.Every rabbit was incised at the backside of its waist,exposing the spinous processes the vertebral plates and the upper-inferior articular processes,entirely segregating the muscles that cohere the spinous processes the vertebral plates and the articluar process,then excising the supraspinal ligaments and interspinal ligaments,biting of two sides the inferior articular processes of L5 and L6,in order that resulting in intervertebral destabilization.The preventive group rabbits were injected 0.01 mg/ml TGF-β1 25~30 μl intra-nucleus gelatinosus on L5-6 and L6-7 through posterlor-lateral approach after intervertebral destabilization operation at once.To draw 8 rabbits,cartilage endplate from every groups after three months and six months orderly.All the exemplar hematoxylin and eosin stain were performed.The data were analyzed using SPSS 15.0 statistics software.Results The degeneration of the cartilage endplate of preventive groups,cell distributed uniform and a clear trend line in 3 and 6 months postoperatively.The Mankin was slower(P<0.05).Conclusions Intra-nucleus gelatinosus injection of TGF-β1 can obviate and treat degeneration of the cartilage endplate due to the intervertebral destabilization.
