摘 要:目的评价髁支持钢板治疗股骨髁上、髁间粉碎性骨折的优越性。方法统计分析髁支持钢板固定及其它3种内固定的疗效,按Merchan等功能评定标准评定膝关节功能。结果交叉针固定组失败率:20.0%;膝关节功能:优8.8%,良20.4%,中25.3%,差45.5%。髁动力加压螺钉固定组失败率11.2%;膝关节功能:优20.3%,良32.7%,中26.6%,差20.4%。L型加压钢板固定组失败率18.4%;膝关节功能:优18.8%,良30.4%,中27.6%,差24.6%。髁支持钢板固定组失败率3.4%;膝关节功能:优44.8%,良30.6%,中19.2%,差5.4%。结论髁支持钢板固定治疗股骨髁上、髁间粉碎性骨折固定坚强,失败率低,膝关节功能恢复好,是目前治疗股骨髁上、髁间粉碎性骨折的较佳选择。Objective To evaluate the advantages of condylar buttress plate for the treatment of femoral supracondylax and intercondylar comminuted fractures. Methods The effect of condylar buttress plate was compared with those of other 3 internal fixators. The knee function was evaluated according to Merchan Standard. Results In cross pins group, the failure rate was 20. 0% , the knee function was excellent in 8.8% , good in 20. 4% , fair in 25. 3% , and poor in 45.5%. In condylar compressive screw group: the failure rate was 11. 2% , the knee function was excellent in 20. 3% , good in 32. 7% , fair in 26.6% , and poor in 20.4%. In “L” compressive screw group: the failure rate was 18.4% , the knee function was excellent in 18.8% , good in 30. 4% , fair in 27.6% , and poor in 24. 6%. In condylar buttress screw group: the failure rate was 3.4%, the knee function was excellent in 44.8%, good in 30. 6% , fair in 19. 2% , and poor in 5.4%. Conclusions Condylar buttress plate for the treatment of femoral supracondylar and intercondylar comminuted fractures can provide rigid fixation, low failure rate and satisfactory knee function recovery, which is optimal for supracondylar and intercondylar comminuted fractures.
