5800例冠状动脉造影术分析

摘　　要：目的:评价选择性冠状动脉造影术(CAG)在冠心病(CHD)诊疗中的应用价值.方法:CAG经股动脉及桡动脉途径进行;对我院完成的5800例CAG结果进行分析,并对部分病例进行随访,统计学:计数资料采用构成比法,其比较应用x^2检验.结果:5800例患者冠脉完全正常者1084例(18.7%),冠脉硬化者764例(13.2%),冠脉狭窄者3952例(68.1%),冠脉心肌桥48例(0.8%),冠脉起源异常者60例(1%),典型心绞痛、运动试验阳性者阳性率高.随访3312例患者,时间为1个月到10年,严重病变或其它原因不能行介入手术治疗组与经皮冠状动脉介入治疗(PCI)组比较死亡率有统计学差异(P<0.05).结论:典型心绞痛、运动试验阳性者及单纯ST-T异常及不典型胸痛者均应积极行CAG,CAG安全可靠,为诊断CHD的“金标准”.Objective： To investigate the value of Coronary Angiography（CAG） in diagnosis and treatment of patients with coronary heart disease.Methods： CAG of 5800 cases were performed by Judkin＇s way through femoral or radial artery from 1993 to 2005 in Cangzhou central hospital. The stenosis of coronary artery was measured by the dianmter. Stenosis 〉 50% was defined as significant lesion. Chi - square test was used for analysis of categorical data. Results： Among these patients, 1084 cases were normal, 60 cases were congenital coronary artery anomalous origin and distribution, 48 cases had myocardial bridges. The number of patients with single branch disease,two branches disease and three branch disease was 1748, 1016 and 1188; The involved vesssels were LM 308 cases, LAD 3544 cases, LCX 1288 cases, RCA 2204 cases. The number of A, B and C type lesion was 2116, 3776 and 2412. Follow - up 3312 patients, from 1 month to 10 years, mortalities are statistically different between the group with PCI and the group without PCI for serious lesions and other reasons（ P 〈 0.05） . Conclusion： Trial evidence demonstrates that CAG was the best approach in patients with chest pain and abnormal ECG. It was safe and effective.
