药学服务对糖尿病患者用药依从性和血糖控制的影响

摘　　要：目的:探讨临床药师对糖尿病患者合理用药干预的效果。方法:要求51例依从性差、血糖控制不佳的糖尿病患者参加每月一次的糖尿病防治教育,同时临床药师为每位患者提供见面时、2-3周后、12周后共三次面对面的药学服务,每次时间不少于20 min;给每位患者建立药历,记录个人情况并提供电话咨询服务。患者自身对照,观察12周后治疗的依从性和血糖控制的变化情况。结果:依从率从0周的48．3％±25．6％升高到12周的95．9％±8．2％;HbA1c由0周的7．8％±1．3％下降到12周的7．2％±1．1％(P<0．05)。结论:药学人员开展临床药学实践,为糖尿病患者提供药学服务,使患者用药的依从性和血糖控制都得到了明显改善。Objective： To study the effects of rational drug use of patients with diabetes mellitus intervened by clinical pharmacists. Method： 51 diabetic patients whose both compliance and blood glucose control were poor were required to attend a lecture on the prevention and treatment of diabetes mellitus once a month. Clinical pharmacists provided the face-to-face pharmaceutical care three times during the same period. The first service was provided when the patient met his （her） pharmacist at the first time. The second service was provided at 2-3 weeks later after they met each other. The last service was arranged 12 weeks later. Each service time should be no less than 20 minutes. During this period, pharmacists should establish the individual records of medication for their patients to record the situation and offer the patients consultant service by phone. The observation was conducted on the variation of patient＇s compliance and the level of blood glucose since 12 weeks after the first pharmaceutical care according to self-control. Result： The rate of compliance was from 48.32%± 25.61% at the beginning of the service up to 95.86% ± 8.16% 12 weeks later, while HbA1c decreased from 7.76% ± 1.32% to 7.24% ±1.14% in the same period. Conclusion： Clinical pharmaceutical care performed by the pharmacists can significantly improve compliance with therapy and the control strength of blood glucose of patients with diabetes mellitus.
