邓老冠心胶囊改善冠心病患者生活质量临床研究

摘　　要：目的观察邓铁涛教授经验方邓老冠心胶囊在提高冠心病心绞痛气虚痰瘀型患者生存质量的临床疗效。方法选择符合WHO标准,至少有4个月以上典型劳力型心绞痛患者共93例,随机分组,分别用邓老冠心胶囊、消心痛及复方丹参滴丸治疗,疗程6个月。观察对心绞痛症状、生活质量等的疗效。结果3组均能显著改善心绞痛症状、硝酸甘油停减率、心电图疗效及动态心电图疗效,但冠心胶囊治疗组的硝酸甘油停减率及改善中医综合征候疗效显著(P<0.05);冠心胶囊组能显著提高患者在一般健康状况、精力、情感职能、精神健康及健康变化方面的得分(P<0.05),而在生理机能、生理职能、躯体疼痛方面,3组间无明显差异;冠心胶囊组在治疗满意程度方面得分与消心痛、复方丹参滴丸组相比有显著差异(P<0.05)。结论邓老冠心胶囊是治疗冠心病心绞痛气虚痰瘀型的有效中成药,能减少心绞痛的发作,提高患者的生存质量,较消心痛更适合长期服用。临床上,SF-36量表及SAQ量表可以作为评价中成药治疗冠心病心绞痛疗效的有效手段。[Objective] To evaluate the efficacy of Dr. Deng＇ s CHD Capsules for the improvement of quality of life of CHD patients of the qi deficit plus phlegm and blood coagulated class. [Method] According to the WHO＇ s diagnosis criteria, a total of 93 stable angina patients were split into 3 groups using single blinding method. The groups were evenly separated into Dr. Deng＇ s CHD Capsule treatment group, XXT controlled group I, and FFDS controlled group II. Two courses of treatment which lasted for 6 months were undergone. During the courses of treatment, the following were observed to assess patients＇ quality of life： clinical symptoms of angina, EKG reading, survey SF-36 of the patients＇ health, and Seattle＇ s angina SAQ indicator. [Results] After 6 months treatment, all three group have good curative effect at angina pectoris and EKG and active flatbed motion test. Their differences had no statistical significance. The treatment group showed a better result in nitroglycerin stopped or decreased percentage and Chinese medical symptoms than the others （P 〈0.05）. The general health condition, energy level, sentiment functional authority and psychological health and health change and treatment satisfaction of the treatment group were significantly better than those of controlled groups Ⅰ and Ⅱ. The scores of physiological functions and physiological functional authority and body pain were not different between three groups. [Conclusions] Dr. Deng＇ s CHD capsules is effective in the treatment of CHD angina pectoris of the qi deficit plus phlegm and blood static class while improving the quality of living. Dr. Deng＇ s CHD Capsules is more adapt to be used in long-term treatment than isosorbide mononitrate. The 36 items short form of health survey and Seattle Angina Questionnaire can be used to appraise the curative effect of Chinese traditional medicine patent medicine for CHD angina pectoris.
