全病毒流感灭活疫苗免疫效果及接种反应观察

摘　　要：目的:为了解国产长生公司全病毒流感灭活疫苗免疫效果及接种反应情况,以评价该公司产品质量。方法:分三年龄组分别采集13～18岁42名,19～59岁33名,60岁以上30名人群免前及免后21天血进行血清血凝抑制(HI)抗体检测,并同时于接种后24小时、48小时、72小时调查调查人群接种反应情况(包括体温、局部红晕、全身症状)结果:105名被调查人群H1N1型阳转率85.7%(90/105),保护率94.3%(99/105),GMT增长倍数18.26;H3N2型阳转率45.7%(48/105),保护率100%(105/105)GMT增长倍数2.37;B型阳转率62.9%(66/105),保护率48.6%(51/105),GMT增长倍数4.81;各年龄组H1N1型阳性率均高于其他两型,而各年龄组H1N1、H3N2型保护率均较高,B型较低。13～18岁,19～59岁H3N2型GMT增长倍数较低,其他组别GMT增长倍数能达到有关标准。所有反应个例除一例局部反应红肿为2.5Χ5.0cm为中度反应,其他均为弱反应。各反应个例均于24小时内达高峰,48～72小时基本恢复。结论:本次疫苗接种对各年龄组人群免疫效果较理想,疫苗接种反应轻微且一过性。Objective： To understand the quality of the whole virus flu vaccine manufactured by Chang Sheng Company by evaluating the immunogenicity and reactogenicity of the vaccine. Methods： Subjects were divided into three age groups. The subjects number were 42 （13-18 years old）, 33 （19-59 years old） and 30 （〉60 years old）, respectively. Serum HI antibodies were tested pre-and 21 days post-vaccination. Body temperatures, local redness and general symptoms were collected on 24, 48 and 72 hours post-vaccination, respectively. Results： For H1N1, the seroconversion rate among 105 subjects was 85. 7% （90/105）, protection rate was 94. 3% （99/105）, GMT increased 18.26-fold. For H3N2, the seroconversion rate was 45. 7% （48/ 105）, protection rate was 100% （105/105）, GMT increased 2.37-fold. For B strain, the seroconversion rate was 62.9% （66/105）, protection rate was 48.6% （51/105）, GMT increased 4. 81-fold. The higher positive rate for H1N1 can be found in each age group. The protection rate for H1N1 and H3N2 were high and for type B was relative lower. The GMT for H3N2 increased lower in 13-18 and 19-59 years groups, and it can meet relevant standard in other group. All reactions were mild reactions, only one subject had local redness with moderate reaction （2.5X5. 0 cm）. All the reactions reached peak within 24 hours and recovered within 48-72 hours. Conclusion： The immunization effect was good for each age group. The reactions were mild and transient.
