慢性乙型肝炎患者外周血树突状细胞亚群相对数量特点的分析

摘　　要：目的分析慢性乙型病毒性肝炎患者外周血树突状细胞亚群(DC1和DC2)相对数量特点。方法采集健康人和慢性乙型肝炎患者外周静脉抗凝全血,利用荧光抗体标记和流式细胞仪检测外周血树突状细胞亚群,DC1的特异性标记为Lineage-HLA-DR+CD11c+,DC2的特异性标记为Lineage-HLA-DR+CD123+;电化学发光检测血清乙型肝炎病毒标志。结果慢性乙型肝炎患者外周血DC2相对数量高于健康人近两倍,但缺少统计学意义,DC1相对数量在两者间没有差别;血清HBeAg阴性慢性乙型肝炎患者外周血DC2相对数量显著增高(P=0.002),差异存在于与HBeAg阳性患者和健康人比较中(分别D1,2=129,P<0.01;D2,3=108,P<0.05),而在上述3组中DC1相对数量无差异(P=0.183)。结论慢性乙型肝炎患者外周血DC2增加与HBeAg血清转换有关,提示DC2可能有抑制病毒的作用。Objective To analysis the percentage of circulating dendritic cells （DC） subsets in patients with chronic hepatitis B. Methods The plasmacytoid DC （ DC2, Lineage^-HLA-DR^＋ CD123^＋ ） and non-IFN-alpha producing myeloid DC （ DC1, Lineage^-HLA-DR^＋ CD11c^＋ ） in fresh peripheral blood from chronic hepatitis B patients without any anti-viral treatment and healthy donors were enumerated by using three-color flow cytometry. The serological markers of hepatitis B virus were evaluated by electrochemiluminescence immunoassay （ECLIA）. Results Percentage of DC2 in patients with chronic hepatitis B was nearly 2 times higher than that of health donors, but without statistical difference. Further, serum HBeAg negative patients with chronic hepatitis B exhibited a significant increase in percentage of circulating DC2 （ P=0.002 in sum）, compared with HBeAg positive patients and health donors （D1.2=129, P〈0.01 ; D2.3=108, P〈0.05, respectively）. The comparisons of DC2 between HBeAg positive patients and health donors showed no significant difference. And there is no significant difference of DC1 in peripheral blood between chronic hepatitis B patients and health donors or among three groups above （ P=0.183）. Conclusion The percentage of peripheral DC2 is associated with the HBeAg seroconversion and may plays a role on inhibition of HBV replication in patients with chronic hepatitis B.
