摘  要:目的探讨肿瘤坏死因子-α(TNF-α)在肾病综合征(NS)发病中的作用,观察黄芪对肾病综合征TNF产生水平的影响。方法104例NS患者在接受强的松和环磷酰胺治疗的基础上随机分为黄芪组和对照组,黄芪组加用黄芪注射液静脉注射,以ELISA检测治疗前及4周后血清及尿TNF-α水平。结果NS患者血清和尿TNF-α水平显著增高(P<0.01),伴轻度肾功能损害患者血清TNF-α水平显著高于肾功能正常者(P<0.01)。4周后NS患者血清及尿TNF-α水平显著下降(P<0.01),黄芪组较对照组变化尤著(P<0.05)。黄芪组的白细胞减少发生率显著低于对照组(P<0.05)。结论TNF-α参与了NS发病过程,黄芪可通过调节TNF-α产生减轻NS的免疫损害,同时可减少环磷酰胺的副作用。Objective To investigate the roles of tumor necrosis factor-α (TNF-α) in pathogenesis of nephrotic syndrome (NS) and the effects of astragalus on TNF-α production. Methods A total of 104 patients with NS treated with prednisone and cyclophosphamide were randomly divided to 2 groups: astragalus group and control group. Astragalus group were treated with astragalus injection. Serum and urinary levels of TNF-α in 104 patients with NS were determined by ELISA at pre-treatment and 4 weeks after treatment with immunotherapy. Results Serum and urinary levels of TNF-α in NS patients were significantly higher than those of healthy individuals (P〈0.01). Serum levels of TNF-α in NS patients with mild azotemia were significantly higher than those in NS patients with normal levels of serum creatinine (P〈0.05). At 4 weeks after treatment with immunotherapy, serum and urinary levels of TNF-α were significantly lower compared with pre-treatment (P〈0.01). Serum and urinary levels of TNF-α in astragalus group were significantly lower than those of control group (P〈0.05). The rate of leukopenia in astragalus group was significantly lower than those in control group (P〈0.05). Conclusion TNF-α may take part in immune pathogenesis of NS. Serum and urinary levels of TNF-α may be a useful indicator for monitoring disease activity and degree of renal damage in NS. Astragalus can contribute to regulating TNF-α production and protecting renal function in NS, and decreases adverse reaction of eyclophosphamide.
