双炔失碳酯合并米非司酮配伍米索前列醇终止早孕的临床研究

摘　　要：目的观察双炔失碳酯与米非司酮抗早孕的协同作用,以及对药物流产的成功率、流产后出血时间的影响。方法210例研究对象随机分为3组每组70例。A组:常规药物流产;B组:常规药物加双炔失碳酯7.5 mg,间隔12 h×4次;C组:在常规药物加双炔失碳酯7.5 mg,间隔24 h×2次。观察3组的胎囊排出时间、流产后阴道出血情况及尿人绒毛膜促性腺激素(hCG)的变化。结果与A组比较,B、C组胎囊排出时间及流产后出血时间均明显缩短(P<0.01),流产后出血时间<14 d者明显增加(P<0.01),>21 d者明显减少(P<0.01),流产后2周尿hCG阳性率均明显降低(P<0.05);B和C组完全流产率均高于A组,但无明显差异(P>0.05);胎囊排出时间,流产后阴道出血天数及流产后尿hCG的变化,B与C组比较无显著性差异(P>0.05)。结论常规药物流产的基础上加用小剂量双炔失碳酯可以加强米非司酮的抗早孕作用,缩短药流后出血时间,提高药物流产的安全性。Objective： To improve the complete abortion rate of drug induced abortion and reduce the side reaction of long time bleeding after drug induced abortion, the synergistic regime of anordrin and mifepristone was studied. Methods： Two hundred and ten women of early pregnancy were randomly divided into three groups, 70 for each group. Women in group A were treated with routine abortion drug （mifepristone plus misoprostol）, those in group B took anordrin 7.5 mg （Q12 h×4） in combination with routine abortion drug and those in group C took anordrin 7.5 mg （Q24 h× 2） in combination with routine abortion drug, The time of fetal sac discharge, post-abortion bleeding and urinary hCG were compared and analyzed. Results： Compared with the group A, the time of fetal sac discharge and post-abortion bleeding in group B and group C were significantly shortened （P〈0.01）. In group B and group C, more women had less than 14 days of post-abortion bleeding （P〈0. 01 ）, and fewer women experienced prolonged post-abortion bleeding more than 21days （P〈0.01）. The positive rate of urinary hCGat 2 weeks after abortion was lower in group B and group C （P〈0. 05）. The complete abortion rate in group B and C was higher than that of group A, but the difference was not significant （P〉0. 05）. There was no significant difference in the time of discharge of fetal sac, the time of post-abortion bleeding and the change of urinary hCG after abortion between group B and group C （P〉0.05）. Conclusion： Small dose of anordrin can intensify the contraceptive effect of mifepristone, shorten the period of bleeding after medical abortion, and improve the safety of the drug induced abortion.
