摘  要:目的探讨经阴道三维超声检查宫内节育器(IUD)的优势及临床意义。方法对要求取出IUD的152例患者,术前分别行经阴道常规灰阶超声及三维超声检查,检测IUD形态、位置及其与宫腔周围的关系,对比分析常规超声及三维超声术前声像图结果并与IUD术后结果相比较。结果 152例患者常规超声检查IUD检出率为86.84%,其中20例IUD显示不清;三维超声检查检出率 100%。三维成像后确诊IUD变形10例、断裂1例,与取节育器术后结果一致。结论三维超声对 IUD的形态及其与宫腔周围的关系判断更准确,有助于制定取IUD手术方案。Objective To evaluate the advantages and clinical value of three-dimensional transvaginal ultrasound imaging for assessment of intrauterine devices (IUD). Methods This series included 152 women [mean age, (35.0 ± 1.9) years; the time after IUD insertion ranged from 3 to 30 yeas] who asked to remove IUD. Preoperatively, they were examined by two-dimensional (2-D) and three dimensional (3-D) transvaginal ultrasound imaging, respectively. The IUD shape, position and its relation with uterine cavity were observed. The ultrasound findings were analyzed and the differences between 3-D and 2-D imaging were compared, and then they were compared with the post-operative findings. Results In the 152 women, the detection rate of 2-D transvaginal ultrasound imaging was 86.84%, with 20 IUD views being vague; while the detection rate of 3-D transvaginal ultrasound imaging was 100%. By 3-D transvaginal ultrasound, 10 IUDs were diagnosed as disfigurement and 1 IUD was diagnosed to be broken. The findings of 3-D ultrasound imaging were consistent with the post-operative findings. Conclusions Stereoscopic images of coronal view can be obtained with 3-D ultrasound, which is helpful for more accurate observation of the IUD shape, location and its relation with uterine cavity. The application of 3-D ultrasound can help choose the plan of removal of IUD.
