摘  要:目的探讨采用声学定量(AQ)技术观察充血性心力衰竭患者左心室功能的改变.方法对34例充血性心力衰竭患者(NYHA心功能Ⅲ~Ⅳ级),采用AQ技术测量左心室舒张末容积(EDV)、收缩末容积(ESV)、射血分数(EF)、峰值排空率(PER)、峰值快速充盈率(PRFR)、峰值心房充盈率(PAFR)及峰值快速充盈率与峰值心房充盈率之比(PRFR/PAFR),并与24例正常人的上述各参数测值进行对比分析.结果心衰组与正常组相比左心室EDV、ESV、PAFR及PRFR有不同程度的增大(P<0.01或P<0.05),EF和PER有不同程度的减低(P<0.01或P<0.05),而PRFR/PAFR差异无统计学意义.结论AQ技术有助于评价充血性心力衰竭患者的左心室收缩及舒张功能,对于晚期心衰患者左心室舒张功能的评价有一定局限性.Objective To evaluate the left ventricular systolic and diastolic function in patients with congestive heart failure (CHF) hy acoustic quantification ( AQ ). Methods This series included 24 healthy subjects (control group) and 34 censecutive patients with CHF (NYHA functional class III to IV) who were diagnosed 13y physical examination, ECG, X-ray and echocardiography. Using AQ technique the left ventricle end diastolic volume (EDV) , end systolic volume (ESV) , ejection fraction (EF) , peak ejection rate (PER), peak rapid filling rate (PRFR) and peak atrium filling rate (PAFR) were measured; and the PRFR/PAFR ratio was calculated. All these parameters were comparatively analyzed with those in control group. Results Compared with healthy controls, EDV, ESV, PRFR and PAFR significantly increased ( P 〈 0.01 or P 〈 0.05) ; EF and PER significantly decreased ( P 〈 0. 01 or P 〈 0.05 ) ; while PRFR/PAFR ratio had no significant differences. Conclusions AQ is helpful to evaluation of left ventricular systolic and diastolic function in CHF patients; while for evaluation of left ventricular diastolic function of heart failure patients at late phase, AQ is limited.
