摘 要:目的探讨建立耳蜗缺血/再灌注动物模型的方法。方法随机将豚鼠分为5组:正常组、手术组、血栓组、溶栓组及对照组。手术经颅底暴露小脑前下动脉(AICA),血栓组使用40%三氯化铁(FeCl3)溶液诱导AICA形成血栓;溶栓组在血栓诱导形成后,经颈外静脉给予尿激酶(UK)溶解血栓;对照组在血栓诱导形成后,给予等量生理盐水。实验过程中采用激光多普勒血流量仪监测耳蜗血流量(CoBF),实验前后测量豚鼠听性脑干反应(ABR)值,血栓组及溶栓组AICA病理切片HE染色观察血栓形态。结果血栓组、溶栓组和对照组实验后ABRⅢ波反应阈均比实验前有明显提高,正常组和手术组实验前后无明显统计学变化。血栓组、溶栓组及对照组在FeCl3诱导后,CoBF值下降至诱导前约30%,溶栓组在用UK后,CoBF恢复到诱导前大约70%。AICA切片HE染色,可见血栓组AICA有明显白色血栓形成,成分为纤维素、血小板及红细胞。溶栓组可见血栓模糊不清,形态较血栓组稀薄。结论应用FeCl3诱导小脑前下动脉血栓形成并用尿激酶溶解的方法,可以制备内耳缺血/再灌注模型,为临床内耳微循环障碍疾病的研究提供较理想的实验动物模型。Objective To explore a new method for establishing an animal model of cochlear ischemia/reperfusion. Methods The guinea pigs were randomized into five groups: normal group, sham operation group, ischemia group, reperfusion group and saline group. Anterior inferior cerebellar artery (AICA) was exposed surgically via a skull base approach. In ischemia group, a small piece of filter paper containing 5μl of FeCl3 was placed upon the AICA to induce thrombus formation. In reperfusion group, animals were injected urokinase (UK) through left external jugular vein after thrombus formation. In saline group, instead of UK, saline injection was applied accordingly. The blood flow of cochlea(CoBF) was monitered continuously with laser doppler flowmeter. Threshold of auditory brainstem response (ABR) was measured before and after every experiment. The AICAs of ischemia group and reperfusion group after operation were resected and pathological sections were perfomed for observation of the details. Results The threshold of ABR in ischemia group, reperfusion group and saline group increased significantly. After the administrasion of FeCl3, the CoBF decreased to 30% of the base level at 30 min. In reperfusion group, the CoBF returned to 70% of the base level after the injection of UK. In the AICA pathological sections of ischemia group, white thrombus composed of cellulose, blood platelets and erythrocytes could be seen. In reperfusion group, the thrombus was thinner and unclear. Contusion The method provides a relatively ideal model for ischemis/reperfusion of the inner ear and this model may be helpful in studying the mechanisms of hearing loss caused by microcirculatory disorders.
