检测宫颈分泌物胰岛素样生长因子结合蛋白-1诊断胎膜早破临床价值的探讨

摘　　要：目的探讨检测宫颈分泌物胰岛素样生长因子结合蛋白-1(IGFBP-1)诊断胎膜早破的临床价值。方法对临床确诊为胎膜早破孕妇56例及无胎膜早破孕妇40例分别采用快速免疫层析法和传统的石蕊试纸(PH试纸)对其宫颈分泌物进行检测。结果IGFBP-1试纸筛检胎膜早破孕妇56例,阳性49例,阴性7例,筛检胎膜未破孕妇40例,阳性0例,阴性40例,其灵敏度87.5%、特异度100.0%,约登指数0.9,阳性预测值100.0%,阴性预测值85.1%。PH试纸筛检胎膜早破孕妇56例,阳性46例,阴性10例,筛检胎膜未破孕妇40例,阳性2例,阴性38例,其灵敏度82.1%,特异度95.0%,约登指数0.8,阳性预测值95.8%,阴性预测值79.2%。结论检测宫颈分泌物中IGFBP-1是诊断胎膜早破的可靠指标,它明显优于传统的生化指标(PH试纸),具有高度的敏感性和特异性,方便、快速,有重要的临床应用价值。Objective：To study the clinical value of insulin - like growth factor binding protein - 1 （ IGFBP - 1 ） in cervical secre- tion in diagnosis of premature rupture of membrane （PROM） . Methods： The cervical secretion of 56 pregnant women diagnosed as PROM and 40 pregnant women without PROM were measured by rapid immune chromatography and litmus paper （pH test paper） . Results： Of 56 pregnant women with PROM, there were 49 positive cases and 7 negative cases; of 40 pregnant women without PROM, there were 0 positive case and 40 negative cases with IGFBP - 1 test （sensitivity 87. 5% , specificity 100. 0% , Youden＇s index 0. 9, positive prediction value 100. 0% and negative prediction value 85. 1% ）. Of 56 pregnant women with PROM, there were 46 positive cases and 10 negative cases; of 40 pregnant women without PROM, there were 2 positive case and 38 negative cases with pH test （sensitivity 82. 1% , specificity 95. 0% , Youden~ index O. 8, positive prediction value 95. 8% and negative prediction value 79. 2% ） . Conclusion： IGFBP - 1 in cervical secretion as a reliable index is a fast and easy method with high sensitivity, specificity, which plays important applied role in clinic.
