MRCP诊断胰头癌的特异征象

摘　　要：目的探讨磁共振胰胆管水成像(M RCP)诊断胰头癌的特异征象,提高胰头癌的确诊率。方法对经手术和病理证实的25例胰头癌患者的术前M R I、M RCP表现进行分析,扫描序列包括SE序列T1W I、FSE序列T2W I、T1W IFS、T2W IFS及M RCP冠状位扫描成像。结果M RCP扫描确诊胰头癌24例,确诊率为96%。25例胰头癌M RCP上均有胆总管和主胰管胰头段破坏残留段信号增高且扩张而不相交征。结论M RCP可用于诊断胰头癌。胆总管和主胰管之胰头段破坏,残留段信号增高且扩张而不相交征是其特异征象。Ohjective： To evaluate the special signs of MRCP on diagnosis of pancreatic head carcinoma. Methods ：To analyze MRI and MRCP features of 25 cases with pancreatic head carcinoma confirmed by operation and pathology,SE sequences with T1WI and fat saturation,FSE with T2WI were performed. Results：Pancreatic head carcinoma were diagnosed correctly by MRCP in 24（96%） cases,and 1 case was misdiagnosed as chronic pancreatitis. MRCP of all cases demonstrated abrupt interruption at pancreatic head segments of dilated left common bile duct and main pancreatic duct at the pancreatic head ones,the left segments showed higher signal were dilated and didn＇t cross each other. Conclusions; MRCP can he used to diagnose the pancreatic head carclnoma,special signs of which are destruction of the pancreastic head segments of the common bile duct and the main pancreastic one, dilated left segments and higher signal and no crosis.
