特发性血小板减少性紫癜患儿外周血T细胞凋亡信号蛋白表达的临床意义

摘　　要：目的检测凋亡信号蛋白死亡结构域(FADD)、Fas、FasL和核因子κB(NF-κB)在特发性血小板减少性紫癜(ITP)患儿外周血T细胞中的表达,探讨其对T细胞凋亡的影响及其临床意义。方法无菌采集35例ITP患儿及30例健康儿童外周血,T细胞分离富聚柱法分离纯化T细胞,流式细胞仪检测T细胞凋亡率及Fas、FasL蛋白的表达;免疫组化法检测FADD、NF-κB P65蛋白的表达。结果ITP患儿T细胞凋亡信号蛋白Fas、FADD表达与正常儿童比较明显下降,FasL、NF-κB P65表达显著上升,差异均有显著性意义(P均<0.05);患儿外周血T细胞凋亡率明显降低,并与血小板计数呈对数正相关(r=0.880 P<0.05)。结论ITP患儿可通过下调引起T细胞凋亡的信号蛋白Fas、FADD表达,同时上调活化增殖蛋白FasL、NF-κB P65表达,导致ITP患儿T细胞凋亡减少、活性增强,引起血小板大量损伤。Objective To investigate the exprestion of apoptotic signal proteins（FADD）, Fas, FasL and NF-κB） in peripheral blood T lymphocytes in idiopathic thrombocytopenic purpura （ITP） children and its correlation with clinical outcome. Methods Collecting aseptic peripheral blood from ITP children （n= 35 ） and healthy children （ n =30）, T lymphocytes were isolated and purified by the T cell Segregation Enrichment Colunm, Fas, FasL and T cell apoptosic ratio were detected by FCM. Inmmnohistochemical staining was used to detect the level of NF-κB and FADD. Results the expression rates of Fas, FADD in ITP children decreased, but the expression rates of FasL, NF-κB increased. The differences between ITP children and heathy children had statistics significance（ P〈 0.05 ）. The apoptosic ratio of T lymphccytes decreased significantly and positively correlated with the platelet count（ r=0. 880 P 〈0.05）. Conclusions T lymphocytes apoptotic ratio in ITP children decreases and its activity enhances, which resulted in immunological function disorder and mass platelets damage by the cooperative function with down-regulation of expressions of the sigmal proteins canses apopto sis such as Fas and FADD and with up-regulation of those, leading to survival and activation, such as FasL and NF-κB.
