肺癌患者支气管肺泡灌洗液CEA、CA125、CA211、NSE检测及其临床意义

摘　　要：目的:探讨肺癌患者支气管肺泡灌洗液(BALF)中CEA、CA125、CA211、NSE浓度对肺癌的诊断、病理分型及分期的意义。方法:选择经组织学和细胞学确诊的60例肺癌患者作为检测组,23例肺部良性病变患者作为对照组,采用酶联免疫吸附法,分别测定2组患者血清及BALF中CEA、CAl25、CA211、NSE的浓度。结果:肺癌组血清及BALF中CEA、CA125、CA211、NSE浓度明显高于肺部良性病变组。两组比较均有显著差异(P<0.05)。BALF中4种肿瘤标志物浓度与肺癌的病理分型无显著差异(P>0.05);Ⅲ-Ⅳ期肺癌BALF中CA211水平明显高于Ⅱ期,二者相比有显著差异(P<0.05),对于肺癌的诊断CA211的阳性率最高,假阴性率最低。BALF中CEA、CA125、NSE与肺癌的分期无相关性(P>0.05)。结论:测定肺癌患者BALF中CEA、CA125、CA211、NSE的浓度对肺癌的诊断有重要临床价值。CA211浓度检测有助于肺癌的分期,而CA125、CEA、NSE浓度检测对肺癌的分期意义不大,检测BALF中4种肿瘤标志物对肺癌病理分型无意义。Objective： To discuss the significance of CEA, CA125, CA211, NSE in bronchoalveolar lavage fluid （BALF） in diagnosis, pathology type and stage of lung cancer. Methods： 60 cases of lung cancer and 23 cases of benign lung disease were enrolled as study group and control group respectively. Enzyme linked immunosorbent assay was used to detect the level of the four tumor markers in serum and BALF in two groups. Results： Levels of CEA, CA125, CA211, NSE in serum and BALF of lung cancr were higher than those in control group, and had significant difference （P〈0. 05）. Levels of four tumor markers in BALF had no correlation with pathology type of lung cancer （P〉0. 05）; CA211 level in BALF of Ⅲ-Ⅳ stage lung cancer was higher than that of Ⅱstage, and had significance different （P〈0. 05）, CA211 had the highest positive rate and lowest false negative rate in lung cancer diagnosis. CEA, CA125, NSE had no correlation with stage of lung cancer （P〉0. 05）. Conclusion：Detecting levels of CEA, CA125, CA211, NSE in lung cancer BALF have significance in diagnosis of lung cancer. CA211 in BALF can help stage of lung cancer, the rest tumor markers had no such sense and detecting all these four tumor markers have no significance for lung cancer pathology type.
