乳腺导管内乳头状瘤癌变18例临床分析

摘　　要：目的:探讨乳腺导管内乳头状瘤(intraductal papillomas,IP)癌变诊断和治疗原则。方法:回顾分析18例IP癌变的临床资料。结果:术前均未能确诊,17例术中冰冻切片考虑IP癌变。10例行腋窝淋巴结清扫术,淋巴结均无转移;1例保乳术后2年复发。结论:术中冰冻切片有助于确诊IP癌变。其治疗选择单纯乳房切除术或保乳手术。Objective： To study the diagnosis and treatment of carcinomatous change of breast intraductal papillomatous. Methods： 18 patients with carcinomatous change of intraductal papillomas were reviwed restrospectively. Results： All of the 10 cases underwent operation. Frozen section showed that 17 cases were considered carcinomatous change of intraductal papillomas. 10 cases with axillary dissection had no metastases to the axillary limph nodes. Local-regional recurrence occurred in one case two years after brest-conserving surgery. Conclusion： Frozen section is helpful for definitive diagnosis. Simple mastectomy or brest-conserving surgery is the good choice in the treatment for the patients with carcinomatous change.
