摘 要:为评价心肌灌注断层显像定量分析多种表示法在诊断冠心病(CAD)中灵敏度、特异性和准确性的差异,对11例正常人和22例 CAD 患者进行了运动态和延迟态(201)~Tl 心肌 SPECT 显像,并均进行了靶心法、圆周剖面法、四片法和四片求积法的心肌显像定量分析。结果诊断 CAD 的灵敏度、特异性和准确性:靶心法为87.5%、88.3%和88.0%;圆周剖面法为93.0%、91.0%和91.8%;四片法为93.5%、92.5%和92.99%;四片求积法为97.6%、95.1%和96.1%。4种表示法的灵敏度、特异性和准确性差异无显著性(P>0.05)。因此,应从所要观察的不同侧面和角度去选择相应的定量方法,才能提高临床诊断能力。PURPOSE We evaluated and compared the sensitivity,specificity and accuracy of four quantitative analytical methods for detecting coronary arterial disease (CAD)using single photon emission tomography (SPECT) with ^(201)Tl.METHODS The data from 11 normal subjects and 22 CAD patients who underwent ^(201)Tl myocardial SPECT were analysed with Polar or Bullseye Map (PM or BM),Circumferential Profile method (CP),4S and 4-SI method.RESULTS The sensitivity,specificity and accuracy for PM method were 87.5%,88.3% and 88.0% respectively;for CP method 93.0%,91.0% and 91.8%;for 4- S method were 93.5%,92.5% and 92.9%;for 4-S1 method 97.6%,95.1% and 96.1%.There was no significant difference in the sensitivity,specificity and accuracy among these quantitative analytical methods(P>0.05).CONCLUSIONS A correct quantitative method for improving ability of detecting CAD should be choosen.
