Budd-Chiari综合征患者肝静脉血同型半胱氨酸浓度的研究

摘　　要：目的:探讨BuddChiari综合征特定的血栓形成部位与高同型半胱氨酸(Hcy)血症的相关性。方法:应用高效液相色谱分析法测定76例BuddChiari综合征患者的肘静脉血、肝静脉血及46例因缩窄性心包炎、肺动脉瓣狭窄等所致的肝淤血患者(对照组)肘静脉血Hcy的浓度,并对其测得数据进行统计学处理。结果:①BuddChiari综合征患者肝静脉血Hcy浓度为(37.71±6.27)μmol/L,肘静脉血Hcy浓度为(20.14±5.78)μmol/L,2组比较差异有统计学意义(P<0.01);②对照组肘静脉血Hcy浓度为(15.80±6.38)μmol/L,与BuddChiari综合征组肘静脉血比较差异有统计学意义(P<0.01)。结论:高Hcy血症是BuddChiari综合征发病的独立危险因素之一,局部Hcy浓度的增高在BuddChiari综合征血栓的发生、发展中发挥重要作用。Objective： To explore the correlation between the hyper-homocysteinemia and the special partial veins thromboiesis of Budd-Chiari syndrome. Method： Seventy-six cases of Budd-Chiari syndrome patients＇ homocysteine concentration of plasma collected from hepatic and cuhital veins were measured using high-performance liquid chromatography; and 46 cases of hepatic congestion patients＇ cubital veins blood, which suffered from con- strictive pericarditis or pulmonary stenosis were taken as control group. And the correlative analyses were carried out by SPSS statistical software. Result： ①The homocysteine average value of hepatic vein blood in Budd-Chiari syndrome patients （37.71±6.27） μmol/L is higher than that of the cubital vein （20. 14±5.78） μmol/L, there was a significant difference（ P〈0.01）;②The homocysteine concentration of the cabital vein blood in control group （15.80±6.38）μmol/L is the lowest, there was a significant difference（ P〈0.01）. Conclusion： The local hyperhomocysteinemia may be one of independent perilous factor of the Budd-Chiari syndrome. The higher homocysteine level in the patient＇s hepatic veins is very important in the development and progression of the Budd-Chiari syndrome.
