摘 要:最近的研究表明内皮祖细胞(EPCs)有助于心血管损伤如心肌梗死(MI)、动脉粥样硬化(AS)、缺血性心脏病(IHD)、经皮腔内冠状动脉成形术(PTCA)等后的成体新生血管形成和心血管重建。EPCs移植输入可以增加血管形成及心肌细胞再生。在几种实验动物模型中EPCs可促进外周血管或心肌缺血后的血管形成和毛细血管密度及心肌修复。并且,MI后移植EPCs增强心脏功能。这表明祖细胞极有可能成为一种改进治疗这些血管疾病发生、心肌再生、防止再狭窄、抑制新生内膜过度增生的方法。现综述有关祖细胞移植术所获得的观察结果,并探讨通过药物调控如过氧化物酶体增殖因子活化受体(PPARγ)及其兴奋剂噻唑烷二酮(TZDs)等这些新的干预措施以增强EPCs功能和数量,提高其移植效果。Myocardial infarction ( MI ) is a leading cause of cardiac mortality and morbility. In survivors, resultant cardiac remodelling predisposes to ventricular dysfunction and heart failure. Currently, the patients who have significant multivessel coronary artery disease ( CAD ) are at high risk for MI, and benefit most from complete revascularization. Conventional revascularization ,either CABG or PCI, requires a viable myocardium,and target vessels amenable to coronary intervention. Furthermore, arterial remodelling with resultant intimal hyperplasia and restenosis limit the long-term effectiveness of coronary interventions. A large number of patients with CAD remain non-revascularizable and are candidates for new therapeutic modalities. One such approach is cell transplantation. Recently, endothelial progenitor cells have been utilized for cell therapy. Transplantation of EPCs ( a stem cell population derived from bone marrow) reduces neointima formation after vascular injury, improves blood flow in ischemic limbs,and preserves left ventricular function after MI, AS and CHD. Thiazolidinediones are PPARγ agonists that are clinically used to augment insulin sensitivity and counter insulin resistance in patient with type Ⅱ diabetes mellitus. However, accumulating evidence suggest that they exert pleotropic effects on cardiac structure and function. In vivo studies PPARγ and thiazolidinediones treated EPCs will contribute reendothelization and myocardial revascularization. The series of experiments show that PPARγ and thiazolidinediones can impove EPCs function, mobilization, migration,homing, proliferation and differentiation,
