体外转流或非体外转流冠状动脉搭桥手术的质量评估

摘　　要：目的对比评估非体外循环冠状动脉搭桥术(OPCAB)与传统冠状动脉搭桥术(CCABG)的吻合口质量。方法采用手术中的实时定量血流测定,对CCABG与OPCAB中质量控制进行对比。结果人均远端吻合数,CCABG组(3·78±1·11)支,OPCAB组(3·83±0·93)支,再血管化指数CCABG和OPCAB组分别为1·01±0·08和1·10±0·09。所有桥血管的血流均满意,搏动指数(PI)均小于5,各搭桥部位的血流波形未见明显区别,两组平均血流量和PI值差异均无显著性。结论OPCAB可以达到与CCABG同样的桥血流量,说明OPCBA可以达到与CCABG同样的吻合质量。Objective To compare the anastomosis quality of coronary artery bypass grafting （OPCAB） under off-pump with conventional on-pump coronary artery bypass grafting（CCABG）. Methods Comparative analysis of clinical results was made in 100 patients of OPCAB and 100 cases of CCABG by meams of Medi-Stim Butterfly Flowmeter measurement of the grafts flow. Results The mean number of the distal anastomosis of CCABG group was 3. 78＋1.11 ,and that of OPCAB group 3.83＋0.93 . The index of completeness of revaseularization of CCABG group was 1.01＋0.08, and that of OPCAB group 1.10＋0.09. The flow of grafts was satisfied in all patients. The PI values were all under 5. There was no significant difference in the mean flow of grafts and PI value between two groups. Conclusion OPCAB can provide the same flow of grafts and similar completeness of revascularization as CCABG indicating the similar anastomosis Quality of grafts in OPCAB and CCABG.
