摘  要:目的:用Meta分析的方法评价18FDG-PET对非小细胞肺癌纵隔淋巴结转移诊断准确性的价值。材料和方法:搜索Pub/Medline数据库及中文数据库,检索所有用18FDG-PET对非小细胞肺癌纵隔淋巴结转移进行诊断的文献,提取相关文献数据;对文献进行质量评估。数据分析采用Excell2003和SPSS11软件,进行异值性检验,得到合并的诊断敏感性和特异性,合并的比数比(OR)值,和并拟然比(SLR)。结果:共检索到文献33篇文献有18FDG-PET技术,文献质量水平为1-3级(2)。利用随机效应模型进行合并处理,18FDG-PET的合并敏感性为0.883(CI:0.864-0.900),合并特异性为0.919(CI:0.908-0.928),合并阳性拟然比(SPLR)为10.2837(CI:7.644-13.834);合并阴性拟然比(SNLR)为0.172(CI:0.136-0.216),合并比数比(DOR)为68.532(CI:44.410-105.76);加权SROC曲线下面积为0.9539,SE为0.0093,Q*值为0.8958,SE0.0129。结论:入选文献均有较高的水平。显示18FDG-PET对非小细胞肺癌纵隔淋巴结转移诊断有较高的准确性。Objective: To summarize the accuracy of 18FDG-PET for dia gnosis of mediastinal lymph node metastases in nonsmall cell lung cancer. Mate rials and Methods: 33 published studies were identified by Pub/Medline and Chine se Databases to search mediastinal lymph node metastases in nonsmall cell lung c ancer using 18FDG-PET (2-[18F2]-fluoro-2-deoxy-D-glucose positron emission tom ography); A standard extractive data were obtained. Using software Excell2003 an d MT, the Summary DOR , Summary sensitivity and specificity and Summary Likeliho od ratio were caculated. Results: 18FDG-PET summary sensitivity and specificity were 0.883(CI: 0.864-0.900)and 0.919 (CI: 0.908-0.928), respectively; Summary Diagnostic Odds Ratio(Random effects model) was 68.532(CI:44.410-105.76 ); Weig hted AUC(area under curve) was 0.9539,SE0.0093. Conclusion: The papers were re latively good qualities. 18FDG-PET have high accuracy for diagnosis of mediasti nal lymph node metastases in nonsmall cell lung cancer. and is a good modality f or patients, especially 18FDG-PET.
