APACHE II评分在COPD呼吸衰竭治疗中的应用评价

摘　　要：目的评价急性生理学与慢性健康状况评分II(APACHEII)对慢性阻塞性肺病(COPD)的预测效果及适用性。方法收集整理100例COPD呼吸衰竭患者的临床资料,比较存活组和死亡组的APACHEII评分,利用APACHEII评分计算预测死亡人数,并与实际死亡人数进行比较,分析预测模型的受试者工作特性曲线(ROC)下面积。结果死亡组的APACHEII分值显著高于存活组,不同分值段的预测死亡人数和实际死亡人数无显著性差异,ROC曲线下面积0.93。结论APACHEII评分适用于COPD呼吸衰竭患者。Objective To evaluate the predictive effect and applicability of the Acute Physiology and Chronic Health Evaluation Ⅱ（APACHE Ⅱ） in chronic obstructive pulmonary disease. Methods The clinical data of 100 COPD patients were collected and assessed with APACHE 1I scoring system, and the score of survival group was compared with that of death group. The practical death toll was compared with the prognostic death toll caculated by the APACHE Ⅱ score and the areas under of the ROC was analyzed as well. Results The APACHE Ⅱ score of died was much higher than that of survivals. No significant differences were noted between the prognostic death toll and the practical . The areas under of the ROC of APACHE Ⅱ were 0.93. Conclusion The study shows that APACHE Ⅱ is applicable to COPD patients with respiratory failure.
