HCV RNA定量与丙型肝炎临床及病理的关系

摘　　要：目的 探讨HCV RNA定量与丙型肝炎临床及病理的关系及判断疗效。方法 采用美国PE公司生产的5700型荧光定量PCR仪，对665例丙型肝炎病毒单纯感染的慢性肝炎及肝硬化患者行HCV RNA定量检测，对部分患者行血生化及肝穿刺病理检查，用不同类型干扰素治疗。结果 慢性丙型肝炎的HCV RNA定量值比肝炎肝硬化高（P〈0．001）。20例患者的HCV RNA定量与肝脏病理分析，HCV RNA定量与肝组织炎症及纤维化无明显关系。但肝组织炎症程度与血清生化指标有一定相关性，血清丙氨酸转氨酶的高低与肝组织的炎症程度成正相关。32例用不同干扰素治疗，疗效与HCV RNA值可能有关，HCV RNA值越低，效果越好。结论 HCV RNA定量慢性丙型肝炎比肝炎肝硬化高，HCV RNA定量与肝组织炎症及纤维化无明显关系，干扰素治疗疗效与HCV RNA值可能有关。HCV RNA定量是丙型肝炎诊断和观察抗病毒治疗的一项可靠指标。Objective To study the relationship between the quantitative level of HCV RNA and Clinical feature and Pathology of Hepatitis C, and assess curative effects in antiviros therapy. Methods Serum levels of HCV RNA of 665 chronic hepatitis and liver cirrhosis patients who simply infected HCV were quantified detected by PCR - fluorescence. Liver specimens were taken from partial patients for pathological examination. Biochemical indexes were detected in partial patients. 32 patients accepted antiviros therapy with interferon. Results HCV RNA levels of chronic hepatitis C patients were higher than that of liver cirrhosis patients（ P 〈 0.001） .To analysis the HCV RNA levels and liver pathological features of 20 patients,there was no significant correlation between the HCV RNA levels and liver inflammaiton and fibrosis, and the serum level of ALT was positively correlated with the grade and stage of liver inflammation. Tlie eruative effects of 32 patients and accepted interferon might be negatively correlated with the HCV RNA levels. Conclusion Quantiative level of HCV RNA was a reliable index for diagnosis of hepatitis C and assessment of curative effects in antivirus therapy.
