摘　　要：目的:评价抗抑郁药圣约翰草提取物对肠易激综合征(IBS)患者自主神经功能的影响。方法:选取30例IBS患者和20例健康女性为试验组和对照组,独立进行汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)的心理测评,并在4℃的冷水浸手及60mmHg压力下直肠扩张,进行扩张前后心率变异率(HRV)的短程频域分析;试验组服圣约翰草提取物300mg,tid,治疗8周后重复上述测试。结果:试验组的焦虑和抑郁评分均显著高于对照组(P<0.01);冷水试验后、直肠扩张前后试验组的短程频域分析中低频/高频比显著降低(P<0.01),提示患者的迷走神经活性降低,而交感神经张力增强。圣约翰草提取物治疗后,试验组的焦虑和抑郁评分均显著下降(P<0.01),而在冷水试验及直肠扩张前后的低频/高频比显著升高(P<0.05)。结论:圣约翰草提取物治疗IBS时可改善患者的心理状况及自主神经的反应性。Objective： To assess effects of the antidepressive agent St. John＇ s Wort Extract on autonomic nervous functions in patients with irritable bowel syndrome （IBS）. Methods：30 women with IBS,who were orally administered with St. John＇s Wort Extract 300 mg three times a day for 8 weeks, were compared with 20 healthy women in regards to the autonomic nervous functions using Hamilton Anxiety Scale （HAMA） and Hamilton Depression Scale （HAMD）, respectively, prior to and post the 8-week treatment. The frequency bands associated with 5-minute-short-time heart rate variability （HRV） were analyzed before and after these women received a cold-water test and a rectum dilatation along with the anxiety and depression evaluation. Results：Compared with the healthy women, the IBS patients showed significantly higher score in HAMA and HAMD prior to the treatment （ P 〈 0.01 ）. The lower ratio of low frequency band/high frequency （L/H） band and reduction of the high frequency band were observed in the IBS patients after the cold water test, and before and after the rectum dilatation, suggesting the vagal hypofunction and sympathetic hyperfunction prior to the treatment. Both HAMA and HAMD scores were significantly reduced at the end of the treatment, compared to the pre-treatment in the IBS group （P 〈 0.01 ）. A significant increase in the L/H ratio after the cold water test, and before and after the rectum dilatation was found post the treatment compared to the pre-treatment （ P 〈 0.05） ; an increase in the high frequency band was found. Conclusion： The St. John＇s Wort Extract can significantly lower the stress of the autonomic nervous system while it improves the psychological status in patients with IBS.
