高原地区异位妊娠的诊治体会

摘　　要：目的探讨腹腔血自体回输及早期异位妊娠诊断对高原地区异位妊娠治疗和诊断的价值。方法对西藏林芝地区(平均海拔3100m)102例异位妊娠行回顾分析,分为自体输血组及未输血组,分析在该地区诊断及治疗异位妊娠的特点及疾病的转归。结果102例患者均手术治疗,其中52例内出血800～3500ml,实施腹腔血自体回输400～2500ml,均无输血并发症;50例内出血200～700ml,未予输血。本组99例行输卵管切除,3例保留输卵管,均治愈。结论手术为高原地区异位妊娠治疗的主要措施,保守治疗应谨慎进行。高原地区血源短缺,腹腔血自体回输抢救异位妊娠大出血病人具有节约血源、防止疾病传播、减少输血反应、减轻经济负担等优点,值得大力推广。本组102例患者内出血200～3500ml,均未能在异位妊娠早期作出诊断,应努力提高早期异位妊娠的确诊率。Objective To discuss the value of abdominal- cavity-blood autotransfusion and earlier diagnosis of ectopic pregnancy to the therapy and diagnostic of ectopic pregnancy in highland area. Method Review and analys 102 cases of ectopic pregnancy in Linzhi of Tibet ,which has an average 3100m height above sea level.All cases are grouped according to whether they are autotransfusion or not. And analys the feature of diagnosis of ectopic pregnancy in this area and its result. Result All cases are performed operation, 52 cases has a interinal hemorrhage of 800～3500ml and performed 400～2500ml abdominal- cavity-blood autotransfusion. 50 cases has a interinal hemorrhage of 200～700ml and not performed blood transfusion. 99 cases performed fallopian tube resection, 3 cases reserved the fallopian tube, they are all cured. Conclusion Operation is the main method to therapy ectopic pregnancy in highland area. It should be careful for conservative therapy.Highland area has a shortage of blood resource.Abdominal- cavity-blood autotransfusion is worth spreading for it could save blood resource, prevent infectious disease, decrease blood transfusion reaction, and be more economic. All cases are not been diagnosised early, so it should be more quick and accurate for the diagnosis of ectopic pregnancy.
