髋关节发育不全患儿Salter手术前后2D和3DCT的对照分析

摘　　要：目的:应用螺旋CT定性、定量分析髋关节发育不全(DDH)患儿Salter手术前后髋关节的变化。材料及方法:临床及骨盆平片诊断为DDH并行Salter手术的患儿44例,比较其术前及术后半年股骨颈前倾角、髋臼前倾角及轴向髋臼指数等CT资料,经统计学检验。应用3D表面遮盖技术了解手术前后髋臼的股骨头覆盖情况。结果:应用SAS配对t检验,手术前后股骨颈前倾角差值有显著意义,髋臼前倾角差值有明显的显著意义,轴向髋臼指数无显著意义。结论:CT能定性、定量地显示DDH患髋手术前后的变化情况,为观察手术疗效提供了切实的依据。Objective： To analyze qualitatively and quantitatively the change of hip joint before and after Salter operation with spiral CT in children with developmental dysplasia of hip（DDH）. Materials and Methods： Forty-four cases of DDH diagnosed by clinics and plain films were scanned on spiral CT before and after Salter operation. Compared the changes in femeral neck anterversion angle（FNA）, acetabulum anterversion angle（AA） and axial acetabulum index（AAI） before and 6 months after operation. Result： The difference is significant for FNA, obviously significant for AA, and not significant for AAI. Conclusion： CT can qualitatively and quantitatively reveal the change of hips with DDH before and after Salter operation. CT provided a reliable way for clinical follow-up.
