腰椎椎弓崩裂垂直裂隙CT扫描与诊断（附52例报告）

摘　　要：目的：探讨腰椎椎弓崩裂的CT扫描方法，进一步提高对CT征象的认识。方法：采用SOMATOM PLUS 4 EXPERT螺旋CT机和PHILIPS MX8000螺旋CT机，对52例无外伤史的腰椎崩裂并椎体滑脱的患者进行CT扫描。扫描方法均采用扫描层面平行椎间隙扫描及扫描层面垂直裂隙扫描，并进行比较分析。结果：垂直裂隙扫描能清楚显示不规则裂隙的骨质硬化、破碎及宽度，平扫椎间隙扫描显示滑脱的CT征象较多，如椎间盘假性膨出、夹心征、椎体漂移征、叠椎征、双管征等．还能显示椎弓根骨质结构及退行性变的征象。并根据统计说明裂隙宽度与椎体滑脱程度成正比。结论：常规CT扫描加垂直裂隙CT扫描．对本病可提供更多的诊断依据。Objective： To study the CT scan methods for spondyloschisis of the lumabar spine and improve the knowledge of CT features,Methods： 52 cases of spondyloschisis with spondylolisthesis underwent CT scanning using SOMATOM PLUS 4 EXPERT spiral CT and PHILIPS MX8000 spiral CT. The methods included scanning perpendicular and parallel to spondylolistheric fissures. The signs of two kinds of CT imaging were compared.Results：The vertical scan imaging of perpendicular to fissures clearlly show the sclerosis, break zone at the pars interarticular, and the width of irregular fissures. The conventional scan imaging of parallel to fissures show more signs of spondylolisthesis, such as pseudobulging disk, wich sign, floating centrum sign, overlapped vertebrae, double sign, etc, and also show the osseous structures of vertebral pedical and spondylosis signs. Statistics results indicated the degree of vertebrae shift was relative to the width of spondylolistheric fissures.Conclusion： More spondyloschisis signs can be displayed combined conventional CT（parallel） and vertical scan.
