摘 要:目的:探讨高效价乙肝免疫球蛋白(HBIG)阻断乙型肝炎病毒(HBV)母婴传播的作用机理。方法:将93例HBsAg阳性的孕妇随机分成两组,实验组孕妇临产前三个月,每月注射一针HBIG(200IU),共3针,接种后无局部及全身不良反应;对照组不用HBIG。两组孕妇所生婴儿均于出生后4小时内肌肉注射HBIG 100IU 1次,注射乙肝疫苗的时间和剂量均按正常婴儿的操作方案进行,母儿血清HBsAg和抗-HBs用酶免法检测,HBV-DNA用定量PCR检测。结果:实验组婴儿血清HBsAg和HBV-DNA检出率明显低于对照组(P<0.05);抗-HBs阳性率显著高于对照组(P<0.05),实验组阻断效果可达94.7%。结论:孕妇于孕晚期联合应用HBIG和乙肝疫苗进行主、被动免疫,可有效地阻断乙型肝炎病毒母婴传播,降低婴儿HBV感染率。Objective: To explore the effect of hepatitis B immunoglobulin (HBIG) blocking hepatitis B vires (HBV) infection from mother to infant.Methods:93 pregnant women were randomly divided into two groups: the test group was injected with HBIG (200IU) three months before delivery(once each month,totel in three times); the control group was not treated with HBIG.The infants of two groups were injected with HBIG (100IU) after 4 hours of birth and got the hepatitis B vaccine with the time and doses as general way.The HBsAg and HBsAb were detected by ELISA and the HBV-DNA was analyzed with PCR in both of the mothers and infants.Results:The HBsAg and HBV-DNA were found very low in test group infants compared with those in control group (P〈0.05);While the HBsAb was strongly high in test group compared with that in control group(P〈0.05),the blocking rate was high to 94.5%.Conclusion:The Combined application of HBIG and HBV vaccine to pregnant women may effectively prevent the infection of hepatitis B virus from the mother to infant.
