BiPAP呼吸机治疗慢性阻塞性肺病合并Ⅱ型呼吸衰竭的临床疗效观察

摘　　要：目的观察BiPAP呼吸机治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的疗效。方法65例COPD合并Ⅱ型呼衰的患者随机分为对照组32例、治疗组33例,治疗前两组的观察指标〔呼吸频率、潮气量、氧分压(PaO2)、二氧化碳分压(PaCO2)〕比较无显著性差异(P>0·05)。分别给予对照组常规治疗,治疗组常规治疗加BiPAP呼吸机通气治疗,治疗3天后,再测定并比较两组的前述指标,同时观察两组住院期间插管率和死亡率。结果治疗3天后治疗组潮气量、血PaO2明显高于对照组(P均<0·01);PaCO2和呼吸频率明显低于对照组(P均<0·01),两组插管率、死亡率无显著差异(P>0·05)。结论BiPAP呼吸机对COPD合并Ⅱ型呼吸衰竭疗效显著。Objective To observe the curative effect of BiPAP ventilation therapy for patients suffering from COPD with type Ⅱ respiratory failure. Methods Sixty five cases were randomly divided into two groups, 32 cases in the control group and 33 cases in the treatment group.There were no obvious differences between the two groups as for the observation targets before treatment （P〉0.05） .The control group was given routine treatment while the treatment group was given routine treatment together with ventilation therapy of BiPAP respirator. The above targets of the two groups after 3 days treatment were measured and compared. Results Obvious differences between the two groups were observed. Tidal volume and PaO2 in the treatment group were much higher than that in the control group （ P〈0.01 ）, resparation rate and PaCO2 in the treatment group was much lower than that in the control group （P〈 0.01 ） . Rate of dath and tracheal intubation. Were not obvious different between the two groups （P〉0.05） . Conclusion BiPAP ventilation therapy was effective for treating COPD with type Ⅱ respiratory failure.
