老年呼吸衰竭合并营养不良患者匀浆膳的应用体会

摘　　要：目的评价匀浆膳特殊配方对老年呼衰合并营养不良病人的应用效果。方法将24例符合入选标准的老年呼衰病人给予特殊配方的匀浆膳营养治疗,治疗前后分别检测病人的血浆总蛋白(TP)、白蛋白(ALB)、前白蛋白(PALB)、淋巴细胞计数、血红蛋白(Hb)、血胆固醇(TC)、三酰甘油(TG)、氧分压(PaO2)、二氧化碳分压(PaCO2)、氧耗量(VO2)、二氧化碳生成量(VCO2)和呼吸商(RQ)等指标,以评价营养治疗前后的效果。结果应用十天的营养治疗,营养状况明显好转。应用三周后,病人的血气分析指标较治疗前相比有显著性差异(P<0.05或P<0.01)。而且与营养相关的生化指标相比,较治疗前也有显著性差异(P<0.05或P<0.01)。结论早期合理使用人工配制的高脂肪、低糖类的匀浆膳,能降低老年呼衰病人的VO2、VCO2、PaCO2和RQ,对改善呼衰病人的肺功能和营养状况具有一定的作用。Objective Evaluate the actual effects of the special formula ill Even paste meal on people who suffer from respiratory failure and Malnutrition. Methods 24 eases who met the requirements were treated with the special-formula Even paste meal. The indices of plasma total protein （plasma TP）, ALB, PALB, Lymphocyte Amount, Hb, TC, TG, PaO2, PaCO2, VO2（the eomsuption of oxygen）, VCO2（the generation of carbon dioxide） and RQ were checked before and after treatment in order to evaluate its effects. Results 10 days later, the nutrition condition of the patients were improved significantly; 3 weeks later, both the blood-gas analysis indices （P 〈 0.05 or P 〈 0.01） and biochemical indices （P 〈 0.05 or P 〈 0.01） of the patients have been remarkably improved compared with those before treatment. Conclusions The proper use of man-made Even paste meal, which is high in fat and low in sugar at an early stage, can reduce the amount of VO2, VCO2, PaCO2 and RQ in patient＇ s body, and can help to improve their lung funetious and nutrition condition.
