摘 要:目的:观察吸附有骨形态发生蛋白及碱性成纤维细胞生长因子的异体脱抗原松质骨植入股骨头坏死病灶清除区后的病理变化过程,评价其诱导成骨活性及生物降解性,以探索股骨头坏死治疗的新方法。方法:实验于2003-07/2004-07在昆明医学院动物实验中心。实验动物为健康成年杂种犬30只60侧股骨头,随机分为3组,分别为空白对照组;植入异体脱抗原松质骨组;植入复合物组。每组20侧股骨头。建立液氮冷冻诱导性犬股骨头坏死缺损坏死模型,用温盐水复温后,空白对照组不充填植入物,植入异体脱抗原松质骨组充填异体脱抗原松质骨,植入复合物组植入吸附骨形态发生蛋白及碱性成纤维细胞生长因子的异体脱抗原松质骨。其中骨形态发生蛋白12.5mg,碱性成纤维细胞生长因子2000IU。术后3,6,12周分批处死。动物处死后,解剖双侧股骨头以体积分数为0.1的甲醛固定。摄X射线片,行大体观察,光镜下组织病理学观察。结果:所有实验动物无死亡,30只犬共60侧股骨头均纳入结果分析。①空白对照组3,6周时缺损清晰可见,缺损被纤维组织部分充填,无明显炎性细胞浸润;12周时缺损仍然存在,缺损区仍为结缔组织充填,缺损与周围骨组织界限清晰。②植入异体脱抗原松质骨组,3周时纤维组织长入移植物孔隙内,间充质细胞少;6周时移植物吸收不全,表面有新骨形成;12周时移植物完全吸收,可见新生的稀疏骨组织。③植入复合物组3周时肉芽组织和新生血管生长活跃,出现多量的间充质细胞,新骨开始形成,移植区密度低于周围骨组织;6周时移植区与宿主骨分界模糊,移植物完全吸收,大量新生骨小梁和新骨形成;12周时移植区与宿主骨密度相当,与宿主骨界限不清,新骨改建接近正常松质骨。结论:异体脱抗原松质骨是生长因子的良好吸附载体,具有较好的生物相容性和生物可降解性,适宜新组�AIM: To observe the pathological changes after bone morphogenic protein-basic fibroblast growth factor-adsorbing allogeneic antigen-extracted cancellous bone (BMP/bFGF/AACB) being infilled into the necrotic areas of the femoral head, and evaluate their osteoinduction and biodegradation so as to find a new therapy for femoral head necrosis. METHODS: From July 2003 to July 2004, the study was completed in the Animal Experiment Center of Kunming Medical College. Sixty femoral heads of 30 healthy adult crossbred canines were randomly divided into 3 groups with 20 femoral heads in each group: blank control group, AACB group and BMP/bFGF/AACB composite group. The defective and necrotic areas of femoral heads in canines that had been frozon by liquid nitrogen and warmed up by warm saline water were infilled with nothing, AACB or BMP/bFGF/AACB respectively. The dosage of BMP was 12.5 mg and the dosage of bFGF was 2 000 IU. The canines were killed respectively at 3, 6 and 12 weeks after operation, and then the femoral heads were extracted and fixed in 0.1 volume fraction formalin. Roentgenography and gross observation were performed, and histopathological changes were observed under light microscope. RESULTS: No experimental animals died, and all the 60 femoral heads of 30 canines had been brought into the analysis of results. ①In the control group, the defects were distinct and infilled partly by fibrillar tissue which had no reactions of toxicity and inflammation by the end of the 3^rd and 6^th weeks. The defects were still infilled by fibrillar tissue and the boundary between the fibrilar tissue and the host bone was distinct by the end of the 12^th week.② In the AACB group, fibrillar tissue elongated into the grafting material, and there were a few mesenchymal stem cells at the 3^rd week. At the 6^th week, the AACB was degradated partly and new bone formed on the surface of AACB.
