冰石愈伤软膏促进小面积Ⅱ度烧伤创面的愈合

摘　　要：目的:临床验证冰石愈伤软膏治疗小面积Ⅱ度烧伤的疗效及药物的安全性。方法:选择2001-04/2002-12第四军医大学西京医院烧伤外科连续住院总面积≤10%Ⅱ度烧伤患者99例。随机分为试验组74例,对照组25例。试验组用冰石愈伤软膏,按每100cm2涂抹5g药量计算,清创后,将药膏均匀涂抹在单层纱布上,贴在创面上,酌情包扎或半暴露;每3日换药1次;共用药2周(浅Ⅱ度)或4周(深Ⅱ度)。对照组用京万红软膏,用药方法同冰石愈伤软膏。比较试验组与对照组总体疗效;固定时间愈合率,创面愈合时间;用药前后创面细菌定量分析比较两种药物的抑菌效果;试验组用药前后血铅分析检测试验药物的安全性。结果:99例患者均进入结果分析。①两组患者总体疗效情况比较:试验组用药后1周治愈率、总有效率与对照组无显著差异,但2周时实验组总有效率显著高于对照组(95.9%,92.0%,R试=0.5250,R对=0.3816,u=2.1468,P<0.05)。②两组创面愈合时间和固定时间愈合率比较:实验组创面愈合时间短于对照组[(13.85±4.32),(17.04±5.98)d,t=2.858,P<0.05];实验组7d和15d愈合率显著高于对照组[(51.26±25.93)%比(28.60±30.91)%,t=3.590,P<0.01;(94.39±11.26)%比(75.48±24.19)%,t=5.282,P<0.01]。③试验药物与对照药物的抑菌效果:两组均具有较好的抑菌效果,无显著性差异。④安全性分析:试验组用药前后血铅和24h尿铅比较无显著性差异。3例创面用冰石愈伤膏后出现一过性疼痛,未发现其他不良反应。结论:冰石愈伤软膏应用于小面积Ⅱ度烧伤,可促进烧伤创面愈合,未发现不良反应和毒副作用,说明该制剂治疗小面积Ⅱ度烧伤安全有效。AIM： To verify the effect and safety of Bingshi Yushang （BSYS） ointment on small-area degree Ⅱ burn wound. METHODS： Ninety-nine patients with 10% or less degree Ⅱ burn wounds hospitalized consecutively in the Department of Burn Surgery, Xijing Hospital of Fourth Military Medical University of Chinese PLA from April 2001 to December 2002 were recruited . Patients were randomly divided experimental group （n=74） and control group （n=25）. BSYS ointment was used on the patients in the experimental group, with the dosage of 5 g per 100 cm^2. After cleaning the wound, BSYS ointment was evenly layed on monolayer pledget , adhibiting to the wound , binging up or half-exposuring according to the conditions ; Change the drugs every three days; totally for 2 weeks （ light Ⅱ° or 4 weeks （deep Ⅱ°）. Jinswanhong（JWH） ointment was used in the control group , and the use method of the ointment was the same as BSYS ointment.General curative effect, healing rate of fixed time and healing time of the wound were compared between experimental group and control group; Quantitative analysis for wound bacteria were performed before and after the applications of BSYS or （JWH） ointments to compare the bacteriostatic effects of the two ointments. The lead concentrations in blood and urine were measured respectively to detect the safety of BSYS and JWH ointments application on burn wound. RESULTS： Totally 99 patients entered the result analysis. ①Comparison of the general curative effect of the patients between the two groups ： There was no significant difference of cure rate or general effective rate between experimental group and control group in the 7^th day after burn, while the general effective rate in the experimental group in the 15^th day after burn was significantly higher than that in the control group （95.9% ,92.0%, Rexperiment group=0.5250, Rcontrol goup=0.3816,u=2.1468,P 〈 0.05）. ② Comparison of the wound healing time and fixted time healing rate between the two groups： th
