体外受精-胚胎移植323周期影响妊娠成功的相关因素分析

摘　　要：目的:探讨影响体外受精胚胎移植(IVFET)妊娠成功的相关因素。方法:回顾性分析2000年5月～2003年3月313例323个IVFET周期中患者的年龄、获卵数、卵泡大小等对妊娠成功的影响。结果:右卵巢获卵数明显高于左卵巢(P<0.05)。在输卵管妊娠组和早产/足月产组的卵泡平均直径均明显高于自然流产组(P<0.05);早产/足月产组的宫内膜厚度有高于输卵管妊娠组的趋势(P=0.050);生化妊娠组用FSH支数明显少于自然流产组和胚胎停育组(P<0.01);早产/足月产组用FSH天数明显高于生化妊娠组(P<0.05)。自然流产组的孕妇年龄明显高于早产/足月产组(P<0.05),而且年龄与所有妊娠的卵泡平均直径呈显著负相关(r=-0.238,P<0.05),年龄与取卵数呈显著负相关(r=-0.297,P<0.001)。自然流产组男方平均年龄为(38.25±5.47)岁(n=8)。结论:年龄是决定卵泡大小的重要因素;宫内膜薄不利于胚胎着床;男性和/或女性年龄偏高可能是导致自然流产的重要原因。Objective： To investigate the related factors that affects the outcome of in-vitro fertilization and embryo transfer（IVF-ET）. Methods：Three hundred and twenty three cycles of IVF-ET from 1st May 2000 to 31st March 2003 were analyzed retrospectively. Results：The number of ova retrieved in right ovary was elevated more than that in left one（P〈0.05）. The sizes of average follicle diameters in both oviduct pregnant group and premature and mature delivery group were all significantly bigger than that in spontaneous abortion group（P〈0.05）;the membrane thickness of uterine in premature and mature delivery group was trend to be higher than that in oviduct pregnant group（P=0.05）;the ampoules of used FSH in premature and mature delivery group were significantly increased more than that in biochemistry pregnant group（P〈0.05）. The female age of spontaneous abortion group was significantly higher than that of premature and mature delivery group. The female age was inversely correlated with both size of average follicle diameters in all pregnancy. （r=-0. 238,P〈0. 05） and number of the ova retrieved（r=-0. 297,P〈 0.001）. The average age of spouses of spontaneous abortion group was 38. 25±5.47 years old（n =8）. Conclusion. Female age is an important factor that affects the size of follicles. And the thin membrane of uterine would be not beneficial to implantation. The higher male and/or female ages would be critical effects to result in spontaneous abortion.
