Evaluation of Left Ventricular Systolic Function by Echocardiographic Border Detection:Contrasted with Electromagnetic Flowmetry and Cineventriculography

摘  要:为了证实超声心动图自动边缘检测(AutomatedBorderDeteciton简称ABD)容积测定评价心功能的准确性,我们在4条开胸犬左室模型中,通过增加或减少前负荷。增加后负荷或用药物增加和降低心肌收缩力调控心输出量,进行ABD与创伤性检查的对比研究发现:ABD测定的左室容量各项指标(CO、EF、dV/dTmax等)与电磁流量计所得的心排量,左室造影得到的射血分数以及心导管测定的左室压力微分(-dP/dTmax)相关(r=0.44~0.96,p均<0.05)。其中以ABD测定的心排量与电磁流量计测得的心排量相关最好(r=0.96),ABD测定的dV/dTmax与心导管测定的-dP/dTmax相关较逊。The objective was to determine if echocardiographic automated border detection(ABD)could be used to accurately detect cardiac output(CO),ejecting fraction(EF)and-dP/dT. Apical four chamber view was obtained in 4 open chest dogs. The electromagnetic transducer was placed on ascending aorta. 7F catheter was inserted into left ventricle to perform cineventriculography and record pressure-time curve. CO,EF and dV/ dT measured by ABD were altered by increasing preload or postload,decreasing preload and increasing or decreasing cardiac contractility by pharmacological intervention. The good correlations were obtained between CO, EF measured by ABD and those measured by electromagnetic flowmetry and cineventriculography(r=0.95 and 0.67,p<0.000 1 and 0.001 respectively).The dV/dTmax measured by ABD related to-dP/dTmax measured by catheterization(r=0.44,p<0.02).
