超时限腕关节离断全程康复治疗1例报告

摘　　要：目的:探讨超时限离断再植术后功能康复的有效方法.方法:对1例双腕关节离断再植患儿采取全程功能康复、高压氧治疗、及心理康复相结合的方式进行系统化个体化康复治疗,并对其疗效进行4年随访及分析.结果:患儿双手功能恢复良好,按照手功能评定试用标准达到优.1～5指屈指关节活动度(TAM)达优,恢复为原来关节活动度的80%,拇对掌功能恢复正常,1～5指感觉为S4(浅感觉和深感觉恢复正常,末节两点分辨觉(0.6 cm),外形满意,肌肉无明显萎缩,肌力达V级.结论:成功的手术、高压氧治疗、及心理康复相结合的系统化全程康复治疗是解决超时限上肢离断的有效措施.Objective ： To investigate the valid method to recover the function of the wrists which were replanted over time limit. Methods ： Patient was individually and systematically treated with the whole range rehabilitation combined with HBO and psychotherapy, and the curative effects were observed and analyzed by the follow-up visit. Results ： According to the probation standard of functional assessment, after 4 years, the double wrists function is excellent： the TAM of the fingers is all right, the sensibility is S4, the myodynamia is Ⅴ , the function of the thumb is excellent. Conclusion ： Successful operation and the individual and systematically whole range rehabilitation combined with HBO and psychotherapy treatment is a valid measure to the amputated wrists over time limit.
