CAPD后一次性手术治疗上尿路结石并急性肾后性肾衰的临床研究

摘　　要：为探讨连续非卧床性腹膜透析(CAPD)后一次性手术去除病因治疗上尿路结石并发急性肾后性肾衰的治疗效果,将69例确诊为上尿路结石完全梗阻并发急性肾衰病例随机分成实验组和对照组.实验组34例患者采用CAPD后一次性手术治疗;对照组35例患者中,行血透28例,B超引导下经皮肾穿刺造瘘5例,孤立肾逆行插管引流成功1例.结果表明:CAPD组治愈33例,治愈率97.06%,死亡1例.对照组中治愈18例,治愈率51.43%;无效11例,占31.43%;住院期间死亡5例,进入终生透析1例于出院后1个月死于肾感染,病死率17.14%.CAPD后一次性手术去除病因治疗上尿路结石并发急性肾后性肾衰的方法住院时间短、医疗费用低,是治疗上尿路结石并发急性肾后性肾衰有效方法之一.To explore the curative effects of performing one-off operation after CAPD on the calculus of upper urinary tract with postrenal acute renal failure, 69 cases definitely diagnosed to be the complete obstruction of upper urinary tract with acute renal failure were randomly divided into experimental group and control group. Then, 34 cases in the experimental group were treated by one-off operation after CAPD, and among the 35 cases in control group, 28 cases were treated by hemodialysis; 5 cases by percutaneous nephrostomy, and 1 case was successfully conducted in a drainage through solitary kidney by retro-urethral catheterization. The results showed that in experimental group, 33 cases recovered from the disease and one case ended in death, with a 97.06% cure rate; but in control group, only 18 cases were cured, with a 51.43% cure rate; and 11 cases were of inefficacy, accounting for 31.43%; and 5 cases died in hospital during hospitalization and one case, who needed the hemodialysis for the rest of his/her life, died of kidney infection after he/she had left hospital for one month. Thus it could be concluded that the method- to treat the calculus of upper urinary tract with post-renal acute renal failure by one-off operation after CAPD, cost a lower medical expenditure, with a short time of hospitalization, and was one of effective ways to treat the disease.
