老年人肾癌的治疗与随访(附24例报告)

摘　　要：目的报告24例65岁以上老年人肾细胞癌的手术治疗和随访结果。方法临床分期:Ⅰ、Ⅱ期16例(66.7%),Ⅲ、Ⅳ期8例(33.3%)。术前半数以上患者存在不同程度的心、肺、脑以及内分泌系统合并症。除2例行单纯肿瘤剜除术外,其余均行根治性肾切除术。结果随访1、3、5年,存活率分别为100%、87.5%和66%。各期肾癌手术切除后均能存活1年。Ⅱ期与Ⅲ期比较,3年存活率无差别,5年存活率分别为66%和50%。Ⅳ期3年存活率为0。结论根治性肾切除术对治疗老年人肾癌有实际意义。Objective To report the results of treatment and follow-up of renal cancer in elderly people over 65 years old. Methods In Robson staging, there were 16 cases （66.7%） in stage Ⅰ and Ⅱ , 8 in stage Ⅲ and Ⅳ（33.3%）. Over half of the patients had combined diseases of the circulation, respiration or endocrine system. All patients received radical renoectomy except 2 patients who underwent nephron-sparing surgery. Results When the patients were followed up for 1, 3, 5 years, the rate of survival is 100% , 87.5% and 66% respectively. The rate of 5-year survival is 66% and 50% in stage Ⅱ and Ⅲ, and the rate of 3-year survival has no difference between stage Ⅱ and Ⅲ. The survival rate in stage IV is 0. Conclusion To elderly people with renal cancer, the radical surgery is of clinical significance.
