B超引导Mammotome微创旋切系统在乳腺良恶性疾病中的应用

摘　　要：为探讨B超引导下Mammotome微创旋切系统对乳腺良、恶性病灶的诊断与治疗的应用价值,对121例144处乳腺病灶进行了B超引导下Mammotome微创旋切术,评价其对乳腺病灶的诊治效果。121例144处乳腺病灶均被Mammotome微创旋切切除,平均旋切15次,用时30min,术后病检良性104例,其中乳腺纤维腺瘤78处,乳腺纤维腺病21处,乳腺囊性增生伴大导管扩张12处;恶性17例,其中肉瘤1例,原位癌2例,浸润癌14例。Mammotome切除标本冰冻切片检查的阴性预测值为98·1%(106/108),阳性预测值为了100·0%(15/15),1·5%的错误率。该方法伤口小、创伤小,除4例(3·3%)有轻度皮下瘀血外无其他并发症。初步研究结果提示,B超引导Mammotome微创旋切系统于乳腺良恶性疾病具有较高的诊断价值,可以替代常规手术作为乳腺良性肿瘤和可疑病灶的一种治疗方法。To evaluate the application of ultrasound guided Mammotome minimally invasive biopsy system in the diagnosis and treatment of breast diseases. The operations and biopsy for 144 breast lesions in 121 patients were performed by ultrasound guided Mammotome system , and the diagnostic and therapeutic values of Mammotome were evaluated. 144 lesions in 121 patients were excised by the Mammotome system. The times of incision were 15 and the duration of operations was 30min. There were 78 fibroadenomaof breast.21 fibroadenosisand 12 cystic hyperplasia in 104 cases benign lesions. There were 1 cystosarcomaof breast and 16 breast carcinoma in 17 cases malignant lesions. Application of Mammotome system in the excision of small breast fibroadenoma can be accurately guided by ultrasound. It can also be used to diagnose the suspicious lesions.
