新斯的明拮抗非去极化肌松药的临床效果研究

摘　　要：目的探讨新斯的明拮抗罗库溴胺的可行性。方法选择ASAI^II级,拟在全麻下行择期手术病人40例,随机分成罗库溴胺组(R组)20例,罗库溴胺+新斯的明组(R+N组)20例。麻醉诱导使用罗库溴胺0.15mg/kg静注,术中肌松维持每30min静注罗库溴胺0.08mg/kg。当手术结束前30min停止肌松药,手术结束后R组静脉注射盐水5ml,R+N组注入新斯的明0.04mg/kg及阿托品0.02mg/kg,观察记录各组病人从停止肌松药至肌张力恢复到四个成串刺激(TOF)25%、75%的时间。结果R组肌张力恢复到四个成串刺激(TOF)25%、75%的时间分别为(40.16±7.96)min、(60.59±5.58)min。R+N组为(23.51±3.01)min、(30.95±3.25)min。罗库溴胺组+新斯的明组TOF25%、75%肌张力恢复时间明显短于罗库溴胺组(P<0.01)。结论新斯的明可以早期拮抗罗库溴胺的作用,明显逆转非去极化肌松剂的肌松作用,效果安全可靠。Objective To investigate the reversal effect of rocuronium with neostigmine. Methods Forty patients （ASA class Ⅰ-Ⅱ） scheduled for elective surgery,general anesthesia were randomly allocated to rocuronium group （n= 20） and rocuronium with neostigmine group （n= 20）. All patients were induced with rocuronium 0.15 mg/kg. And maintained with 0.08 mg/kg every 30 minutes. R group patients received neostigmine 0.04 mg/kg with atropine 0.15 mg/kg and R＋N group patients were administrated 5 ml saline. Their reversal effect was compared by 25% and 75 % of TOF. The time from roeuronium administration to recovery of TOF 25 % and 75 % was recorded. Results in R group, the recovery time to TOF 25% and 75% was （40. 16±7.96）min, （60.59±5. 58）min respectively. In R＋N group it was （23.51±3.01）min,（30.95±3.25）min respectively. The recovery time in R＋N group was significant shorter than that in R group （P〈0.01）. Conclusions Recovery time of roeuronium neuromuscular block can be antagonized by early neostigmine administration and That＇s a safe way.
