拉米夫定联合苦参素治疗慢性乙型肝炎的疗效观察

摘　　要：目的:探讨拉米夫定联合苦参素治疗慢性乙型肝炎的临床疗效。方法:将146例慢性乙型肝炎患者随机分成治疗组(拉米夫定联合苦参素治疗)和对照组(单用拉米夫定治疗)各73例,用ELISA法检测HBeAg、抗-HBs、HBeAg、抗-Hbe、抗-HBc,用PCR法检测HBVDNA,观察两组的临床疗效和HBeAg、HBVDNA阴转率。同时观察肝功能及相应临床症状和体征变化。结果:治疗组总有效率为93.15%,显著高于对照组的75.34%(P<0.05);治疗组HBeAg阴转率、HBsAg阴转率、HBeAg/HBeAb转换率和HBVDNA阴转率分别为71.23%、26.86%、54.79%和86.30%,显著高于对照组的50.68%、10.15%、35.62%、61.64%(P均<0.05)。治疗组的肝功能及相应临床症状和体征改善均优于对照组(P均<0.05)。结论:拉米夫定联合苦参素治疗慢性乙型肝炎,是一种安全有效的治疗方法,值得临床推广应用。Objective： To study the therapeutical effect of lamivudine combined with oxymatrine for treatment of chronic hepatitis B. Methods： 146 patients were divided randomly into therapy group （73 patients, lamivudine combined with oxymatrine therapy） and control group （73 patients, lamivudine therapy only）. HBeAg,anti-HBs, HBeAg,anti-Hbe and anti-HBc were detected by ELISA and HBV DNA was detected by PCR. The HBeAg and HBV DNA negativie rates ,liver function,the symptoms and signs of all patients were observed closely. Result：The total effective rates in therapy group and control group were 93. 15% and 75.34% respectively, the difference between the two groups was significant（ P〈0. 05）. HBeAg and HBsAg negative transfer rates, the tranfer rate from HBeAg to HBeAb and HBV DNA negative rate in therapy group were 71. 23%,26. 86%,54. 79% and 86.30%,respectively. In the control group,they were 50. 68%,10.15%,35.62% and 61. 64% ,and the difference of the result was significant （ P〈0.05）. The effects on liver function and symptoms in therapy group were significantly improved compared with those in the control group（ P〈0. 05）. Conclusion：Lamivudine combined with oxymatrine is effective and safe in treatment of chronic hepatits B and worth clinical application.
