2型糖尿病病人微量白蛋白尿有关因素的探讨

摘　　要：目的研究2型糖尿病病人微量白蛋白尿的危险因素。方法80例2型糖尿病患者根据尿白蛋白排泄率(UAER)分为正常白蛋白尿组(NA)、微量白蛋白尿组(MA),分别检测二组的血压、血糖(PG)、血脂(TG、CH、LDL、VLDL、HDL)及血小板α-颗粒膜糖蛋白(GMP-140)、血小板溶酶体膜糖蛋白(LIMP-CD63)表达率,以是否有微量白蛋白尿为因变量拟合多元回归分析。结果MA组的血压、空腹血糖、餐后2 h血糖、CH、LDL明显高于NA组(P<0.01), MA组的GMP-140、LIMP-CD63明显高于NA组(P<0.01);GMP-140、VIDL、FPG、血压水平升高是2型糖尿病病人微量白蛋白尿发生的主要危险因素。结论血小板活化水平升高及长期血压、血糖、血脂控制不良将导致2型糖尿病病人微量白蛋白尿的发生。Objective To explore the related factors for microalbuminana in patients with type 2 diabetes mellitus. Methods Eighty cases of type 2 diabetic patients were divided into two groups according to urinary albumin excretion rate （UAER） ： normoalbuminuric （NA） group ; microalbuminuric （MA） group ; Blood pressure, the expression levels of platelet α - granule membrane glycoprotein （ GMP- 140） and platelet lysosomal granule - membrane glycoprotein （ LIMP - CD63 ） ,and fasting plasma glucose （ FPG）, cholesterol （ CH ）, triglycerin （ TG）, high - density lipoprotein （ HDL）, low - density lipoprotein （LDL） ,very low -density lipoprotein （VLDL） and postprandial plasma glucose （PG2 h） were measured. The multiple liner regression analysis was used with the microalbuminuria as the dependent variable. , Results 1. Blood pressure, FPG, PG2 h, CH, LDL in MA group were significantly higher than those in NA group （P 〈 0.01 ） ;GMP - 140,LIMP - CD63 were remarkably higher in MA group than those of NA group （P 〈 0.01 ）. 2. The increased levels of GMP - 140 ,VLDL,FPG and blood pressure were major related factors for microal buminuria in patients with type 2 diabetes mellitus. Conclusion The increased platelet activation together with long - term uncontrolled blood pressure,plasma glucose and serm lipid participate in the occurrence of MA in type 2 diabetic patients.
