摘  要:目的观察丙酮酸乙酯对脓毒症肺损伤大鼠肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和高迁移率族蛋白B1(HMGB1)表达的影响,探讨丙酮酸乙酯治疗脓毒症肺损伤的免疫调节机制。方法40只Wistar大鼠随机分为正常对照组、假手术组、脓毒症肺损伤组和丙酮酸乙酯(40 mg/kg,每隔6 h腹腔内注射一次)治疗组,每组10只。用ELISA法检测2 h血清TNF-α、IL-1β表达。所有大鼠24 h后活杀,Western blotting法检测肺组织HMGB1的表达,观察PaO2、PaCO2和pH等血气变化以及肺组织的病理变化。结果脓毒症肺损伤组血清TNF-α、IL-1β和肺组织HMGB1表达显著增高,PaO2、pH降低,PaCO2升高,病理显示明显肺损伤(P均<0.01)。丙酮酸乙酯治疗后血清TNF-α、IL-1β和肺组织HMGB1表达显著下调,PaO2、pH升高,PaCO2降低,病理显示肺损伤程度减轻(P均<0.01)。结论丙酮酸乙酯能显著抑制TNF-α、IL-1β和HMGB1等早晚期炎症因子,改善低氧血症,对脓毒症肺损伤有明显保护作用。Objective To investigate the effects of ethyl pyruvate on TNF - α, IL - 1β and High Mobility Group Boxl ( HMGB1 ) expression , and the mechanisms of ethyl pyruvate protecting the lungs against acute lung injury induced by sepsis. Methods 40 Wistar rats were randomly divided into normal control, sham operation , acute lung injury , and ethyl pyruvate treatment (40 mg/kg intra- peritoneally every 6 hrs) groups. Serum was collected to determine TNF - α and IL-1β by ELISA 2h later, At the 24 hours animals in each group were sacrificed, lungs were harvested. The expression of HMGB1 was detected by Western blotting. PaO2, PaCO2 and pH were measured. Histological examination of the lungs was also performed,Results Ethyl pyruvate inhibited significantly TNF - α, IL - 1β and HMGB1 expression( P〈0.01 ) , PaO2 and pH were significantly higher,PaCO2 was significantly lower compared with acute lung injury group ( P〈0.01 ),The histological changes of lung injury were ameliorated. Conclusions This study showed that ethyl pyruvate could inhibit TNF-α, IL-1β and HMGB1 expression,and had protective effects on the lungs against acute injury induced by sepsis.
