摘 要:目的分析子宫腺肌病的MRI和DSA特征,提高对其影像学表现的认识。方法27例子宫腺肌病,TUAE术前行MRI和DSA检查。MRI检查采用SET1、TuborSET2WI和SET1WI增强成像,造影剂为GdDTPA,剂量0.1mmol/kg体重,对不同成像序列的MR信号及相关特征进行分析。双侧子宫动脉超选造影,对不同时相的DSA表现进行分析。结果27例子宫体积均有增大,19例结合带为局限性或弥漫性增厚,平均(36.11±10.50)mm,子宫腺肌瘤8例,体积大小平均为(155.70±81.86)mm3,85%的病例在低信号病变内出现高信号灶(T2WI),有明显强化。27例腺肌病子宫动脉分支均有不同程度增粗、增多、延长,9例有增生血管,与纡曲、增粗的螺旋动脉同时参与供血。结论子宫腺肌病具有典型的MRI和DSA征象,常规MRI能够做出明确的诊断,DSA则能够为其诊断提供更多的信息。Objective To analyze the MRI and DSA feature of uterine adenomyosis, and further understand its imaging apperance. Methods MR imaging and DSA were performed in 27 cases of uterine adenomyosis before TUAE. MR imaging consisted of Tl-weighted spin-echo sequence obtained before and after contrast material administration(0.1 mmol/kg of body weight of gadolinium dimeglumine), T2-weighted Turbo spin-echo sequences. On different sequence, MRI signal intensity and relative features of the adenomyosis were analyzed. Superselective angiography was performed in bilateral uterine artery and the DSA appearances in different periods of time were analyzed. Results All the 27 cases showed uterine enlargment. The junctional zone thickened focally or diffusely in 19 cases, the average thickness was (36. 11 ±10.50) mm. Eight cases showed adenomyoma with the average volume of (155.70±81.86) mm^3. Bright foci, which indicated the vivid enhancement, was seen in the abnormal fields of low signal intensity on T2-weighted images in about 85 % of paients. Uterine arteriograms in 27 cases showed spiral arteries enlarged and lengthened in different degrees. Neogenetic arteries, which provided the blood supply together with the enlarged and sinuous arteries simultaneously, were found in 9 adenmomyoma caes. Conclusion Both typical MRI and DSA fetures were found in adenomyosis patients. Definite diagnosis can be made based on the conventional MR imaging, and DSA can provide more useful information for the diagnosis.
