带蒂瘢痕皮回植在烧伤瘢痕治疗中的应用

摘　　要：目的:探讨带蒂瘢痕皮原位回植在烧伤后增生性瘢痕治疗中的应用、适应证及其优缺点。方法:沿瘢痕纵轴在瘢痕内切口,手术刀锐性剥离,形成0.8～1.2mm厚的瘢痕皮,切除瘢痕皮下的瘢痕组织,将瘢痕皮原位回植,适当加压包扎。5～10天拆线,应用肤康霜或硅酮霜3～6个月。结果:本组28例(52处),其中27例(51处)瘢痕皮均Ι期愈合,局部症状消失,外观满意。其中21例随访4个月～2年,瘢痕未复发,瘢痕皮质地柔软,色泽与正常皮肤颜色相近,患者对治疗效果满意。结论:采用带蒂瘢痕皮原位回植,减少了供皮区副损伤及瘢痕,色泽与正常皮肤颜色相近,不失为烧伤后续治疗的一种有效方法。其缺点是适应证相对较窄。Objective To explore the application and advantage or disadvantage of pedicled scar-skin replant in treatment for hypertrophic scar after burn. Methods Cut the scar along it＇s length with operation knife to form a scar-skin which thickness is of about 0.8～1.2mm,then cut off the scar tissues under the scar-skin,replant the scar-skin in the same place,enswathed the wound with proper pressure.Took out stitches after 5N10 days and applied Centella Triterpenes Cream or Sililonefrost for 3～6 months. Results 27 patients with 51 scar-skin showed satisfactory results with one-stage recover.There were no new scar and the scar-skin was soft with it＇s color and luster the same as normal skin in 21 patients followed up from 4 months to 2 years. Conclusion Pedicled Scar-skin replant is an effective method in treatment of burn which deserves us referring can prevent the normal skin from injury and reduce the number of scar,and the recovered scar-skin is almost the same to the normal skin.But this procedures need some skill which are not as easy as the routine and it does not apply to all the patients.
