摘 要:目的:探讨血清转铁蛋白受体(sTfR)在儿童缺铁性贫血(IDA)诊断中的意义.方法:6月~12岁缺铁性贫血儿童50例及正常儿童20例均进行血红蛋白(Hb)、血清转铁蛋白受体(sTfR)、血清铁蛋白(SF)、血清铁(SI)、总铁结合力(TIBC)测定,并将受测对象分为正常对照组(C组)、轻度IDA组(IDA1组)、中-重度IDA组(IDA2组),以1989年洛阳会议制定儿童IDA诊断标准为据,以不同浓度sTfR为临界点计算其诊断IDA的敏感度及特异度,以确定最佳的诊断临界点.结果:以sTfR浓度 40 nmol·L-1为缺铁性贫血的诊断标准时,其敏感度和特异度分别为90%和85%,为诊断本组IDA敏感度和特异度之和最大的临界点. 结论:当sTfR为40 nmol·L-1 时诊断IDA准确度最高,即为IDA的最佳临界点.Objective: To observe the clinical significance of the serum transferrin receptor (sTfR) in the diagnosis of iron deficiency anemia (IDA) in children. Methods: 20 normal controls and 50 IDA patients aged 6 months to 12 years were involved for the determination of sTfR, Hb and serum ferritin (SF). All the children were divided into three groups: the normal control group (C), mild IDA group (IDA1) and moderate to severe IDA group(IDA2). Among the patients, the sensitivity and specificity was analyzed at the different critical points of the sTfR, to definite the best critical point for diagnosis. Results. When the critical point of the sTfR was 40 nmol·L^-1, the sum of its sensitivity and specificity to diagnose the IDA was the largest, so it's the best critical point to diagnose the IDA. Conclusion: The best concentration to diagnose IDA of sTfR is 40 nmol ·L^-1 , and sTfR is closely related to the severity of IDA.
